QUESTIONS/REQUESTS FOR CLARIFICATION Questions concerning this Invitation for Bids must be submitted in writing to: Thomas P. Watkins at twatkins@salem.com at least five (5) days prior to the bid opening date. Written responses will be mailed to all bidders on record as having picked up the Invitation for Bids. SPECIFICATIONS/SCOPE OF WORK 1. The Contractor shall be available when needed, at any time on short notice. 2. The successful Bidder shall have at least ten years' experience in grave digging. 3. The Contractor shall have a 3' bucket to make an opening 3' wide, 8' long, 5 1/2 ' deep, with square sides and bottom leveled off. (Extra dirt to be hauled away from cemetery property.) 4. The Contractor shall be responsible for any damage caused to lawn, monuments, and during the winter shall remove snow, when necessary, to reach grave. When necessary, he shall also remove monument in order to dig grave. 5. The successful Contractor must complete work within the time constraints specified by the Cemetery Superintendent. Failure to complete work in a timely fashion or to cause delay or to inconvenience the Funeral director or the family of the deceased will be sufficient reason for the City to terminate the contract immediately. 6. The successful Contractor shall be responsible for the following: A. Digging Graves. B. Setting up greens and lowering devices. C. Assisting with flowers and caskets. D. Backfill graves. E. Restoring surfaces, sodding if applicable. F. Repairing all surface damage. G. Cleaning up. 7. The successful Contractor shall be responsible for the following additional items, when necessary: A. Clearing snow. B. Digging through frozen ground. C. Setting up and removing owner furnished tent. D. Placing vault. E. Placing double deep boxes. F. Working overtime. 9. Grave Digging services must be rendered on a seven-day basis. 10. All employees of the vendor shall wear proper attire at all times when providing services to the City of Salem. Such attire may consist of polo shirts identifying company name. All attire shall be approved by the City of Salem Cemetery Superintendent prior to the start of the Contract. 11. No equipment shall be stored on City of Salem property overnight, without prior approval by the City of Salem Cemetery Superintendent. 12. contractor is responsible to remove at their cost spoil piles generated by this contract. To be schedule before completion of this contract. 13. The successful bidder must have or ability to obtain the following equipment to preform all task out lined out in this contract including but not limited to: o Mini excavator o Backhoe with rubberized tires o Six-wheel dump truck with a minimum capacity of 8 yard of material o One-ton dump truck with a minimum capacity of 4 yards of material o Any other piece of equipment asked for by the superintendent of the cemeteries for the city of Salem. 14. The successful bidder must supply a minimum of three refences from preforming similar type of work. 15. The successful bidder must provide a list of current contracts for municipalities.
