Fairview Area Schools Board of Education ("School" or "District") requests written quotes to secure services for resurfacing the existing athletic running track on its primary facility located at the Fairview Area Schools, 1879 E. Miller Road, Fairview, Michigan, 48621 (School). This project includes removal of 1.5" of existing asphalt, installation of new asphalt, sealing, and painting new lines and event markings in accordance with the National Federation of State High Schools Association. SUBMITTAL REQUIREMENTS Vendor shall submit a detailed, Sinking Fund compliant proposal which should include the following minimum requirements: 1. A total project cost proposal which shall include any and all charges involved in the execution of the job, including labor, materials, shipping and handling, material removal, equipment costs, licensing, and any other fees or expenses needed to perform the job. 2. List of all proposed materials along with a description of component materials/specifications. 3. Detailed and specific warranty information for products and workmanship. 4. Indicate anticipated dates of construction and the anticipated completion date. Project must be substantially completed by mid-July, 2022. 5. Copy of current general liability and workman's compensation coverage. 6. Familial Relationship Disclosure - All bids shall be accompanied by a sworn and notarized statement disclosing any familial relationship that exists between the owner(s) or any employee of the bidder and any member of the Board of Education or the Superintendent of the District. The Board of Education will not accept a bid that does not include this sworn and notarized disclosure statement. (Appendix A) 7. Certification of Eligibility by Bidder - All bids shall be accompanied by a sworn and notarized statement disclosing any connections to Iran that exists between the owner(s) or any employee of the bidder. The Board of Education shall not accept a bid that does not include a sworn and notarized Certification of Eligibility Form. (Appendix B) 8. List of client references and/or letters of recommendation are encouraged but not required. 9. Any other information deemed necessary to address the requests of this RFP. This project has been awarded to Reliable Mechanical Systems, Inc. of Traverse City, MI in the amount of $77,500.00.
