The Pike County Engineer and the Board of Pike County Commissioners intend to contract for Structure and Roadway Plan Preparation Services, Right-of-Way Plan Preparation Services and Geotechnical Services in connection with the PIK-CR4/CR50/CR66 Project, PID#115814. The Project Scope Includes: Pike Lake Road, PIK-CR4-3.77-4.68 (0.91 Miles)-Resurfacing including the repair of a 100 ft. long roadway slip with a drilled shaft retaining wall, and upgrading of drainage culverts through the entire project length, Watson Road, PIK-CR50-0.00-3.57 (3.57 Miles)-Resurfacing including the replacement of a failing corrugated metal culvert with a concrete box culvert, and upgrading of drainage culverts through the entire project length, and Germany Road, PIK-CR66-0.14-1.44 (1.30 Miles)-Replacing two undersized and failing culverts with four-sided concrete box culverts including roadway widening and shoulder widening at each location. The following factors will be used by the Pike County Engineer to evaluate each statement of qualifications. Please respond only to the questions listed. Give location of the office where a majority of the work is to be performed and the distance in miles to the site. 3. Detail the project staffing of the office noted in Item #2, broken down by the number of: a. Professional Engineers b. Technicians c. Other Specialists Also indicate the number of such personnel available for assignment to this project. 4. List the project manager(s). Attach resume(s). 5. List the experience of your firm's personnel on similar projects. 6. Provide a representative listing of projects similar to the proposed project performed by your firm. Include number of years your firm has been in business. 7. List your firm's present workload. Please specify the dollar value of PRESENT OUTSTANDING fee agreements. 8. Provide references. 9. Provide a brief description of your firm's equipment and facilities. As required by Ohio Revised Code, Section 153.65-71, responding firms will be evaluated and ranked in order based upon their qualifications. Following this evaluation, the Pike County Engineer will enter into contract negotiations with the most highly rated firm.
