Renovation of a fitness / recreation center in West Peoria, Illinois. Completed plans call for the renovation of a fitness / recreation center.

As of March 29, 2022 this project has not yet been awarded. The project is expected to be awarded on April 13th. The Peoria Park District is soliciting written quotes for the removal and replacement of the tile flooring in the lobby and main level restrooms at Franciscan Recreation Center, located at 908 N. Sterling Avenue, West Peoria, IL 61604. Please note that the prevailing rate of wage including benefits for the Peoria area shall be paid for each craft or type of worker needed to execute this project or perform this work as required by the State of Illinois Department of Labor. Tentative award will be made February 23, 2022. All work must be completed by May 20, 2022. General Description and Scope of Work: A. General: The project's intent is to remove the existing tile, and base trim in the lobby and main level restrooms at Franciscan Recreation Center and replace it with VCT and new vinyl base trim. The space is approximately 620 square feet as shown in the attached drawing. Any measurements provided are for reference only. Contractor is responsible for field verifying all measurements before quote is submitted. No cost-added change orders will be issued for insufficient materials or supplies. B. Products: a. VCT: Armstrong Standard Excelon Imperial Texture, or approved equal. Color to be chosen by Owner's Rep from manufacturer's full range of colors. b. Cove base: Johnsonite Vinyl, or approved equal, 4" tall. Color to be chosen by Owner's Rep from manufacturer's full range of colors. c. Adhesive: Use manufacturers suggested adhesive with each type of product. d. Other materials: Transition strips or other items necessary for a complete installation and smooth transition between new tile and adjacent spaces must be included in quote. C. Execution: a. Park District Staff will be responsible for removing all furniture from areas before work begins. Contractor must provide at a minimum,72 hours' notice in advance of work commencement. b. Remove and legally dispose of or recycle existing tile, vinyl base and all other waste materials generated by this project. c. Contractor is responsible for removing access ramp leading to gym doors, and reinstalling once new tile is in place. d. At closet, tile inside of closet shall remain. Contractor shall add transition strip at door frame between new and old tile. e. Install tile, vinyl cove base and any other necessary materials according to manufacturer's written instructions. f. Site must be kept clean daily. g. Provide Owner with no less than 1 full box of extra tile. h. Before project is handed back to Park District, Contractor shall prep and wax floor with a minimum of 2 coats of wax. Administrative Requirements We are required by State law and Park Board policy to request the following to be submitted with quote. Unfortunately, Quotes submitted without this information are considered incomplete and ineligible for award. All forms noted in this section are available from the Planning Department website. If needed, these forms may be faxed or emailed to you for your use. 1. EEO Certification - A completed Certificate of Equal Employment Opportunity Compliance must be submitted with quotes if not currently approved. 2. Workforce Profile - A completed Workforce Profile must be submitted if not currently approved. 3. Sexual Harassment Policy - A sexual harassment policy must be submitted for approval if not currently on file. 4. W-9 Taxpayer Identification Form: Bidder must complete and sign the W-9 form. 5. Certification of Compliance of the Listed Provisions and Laws- see attached. Required forms after Award is made before work can begin: 1. Insurance Requirements: A Certificate of Insurance and Endorsement with a minimum of one million ($1,000,000) of liability coverage, naming the Peoria Park District as Additional Insured, will be required, before work begins. Verification that all employees who will be working at the site are currently covered by Workers Compensation Insurance will also be required. Forms to be submitted with project closeout and pay requests: 1. Certified Payroll - Prevailing wage is required for this job. Please Note: Illinois State Law has changed. As a Contractor on a public works project, Contractor must submit certified payroll directly to the Illinois Department of Labor and then provide Park District the PDF you receive after submission. 2. Weekly Workforce Reports - Contractor shall submit completed Weekly Workforce Report for each week until project is completed.
