Contractor Awarded Address: Scottsdale, AZ 85255-5697 USA Base and All Options Value (Total Contract Value): $172644.00 pril 20, 2022 FROM: Procurement & Property Services (PPS) Contract Service Area (CSA) 6 - Construction TO: ADW Communications Services, Inc. 18291 N Pima RD STE 110-317 Scottsdale, Arizona 85255 SUBJECT: USDA - Forest Service, 127EAW22C0010 - PNF Pike Co Lookout Solar - Award Notification Acceptance is made of your proposal submitted on March 15th, 2022, in the total amount of $172,664.00 to perform the PNF Pike Co Lookout Solar project located on the Plumas National Forest. 127EAW22R0012 - PNF Pike Co Lookout Solar Plumas National Forest FAR 32.204 Disclosure of Magnitude of the Construction Project: Between $100,000 and $250,000 Set Aside: Total Small Business Set Aside IMPORTANT NOTES: See Section L, Paragraph L.2 for the Documents to Submit with Your Proposal All Requests for Information (RFI's) shall be submitted to the Contracting Officer, Greg Cunningham, on the attached RFI form not later than 1300 hours PST, March 1st, 2022. An organized pre-proposal site visit will be held. See Provision 452.237-71 on page 33 for complete details. In no event shall failure to inspect the site constitute grounds for a claim after contract award. SCOPE OF CONTRACT The contractor shall provide all management, supervision, labor, personnel, transportation, materials, equipment, and supplies, except as otherwise specified, necessary to provide solar design and installation services on the Feather River Ranger District located at Pike County Peak. All work shall be performed in accordance with this statement of work and all federal, state and local laws and regulations. The Contractor should refer to the SCHEDULE OF ITEMS and project plans and specifications when making estimates of quantities or work. PROJECT LOCATON The Lookout and repeater can be accessed from Oroville by travelling on CA-162E for approximately 5 miles before turning onto Forbestown Road and following for approximately 17 miles. Turn left onto LaPorte Rd and follow for approximately 3 miles. Turn right on Forest Service Rd 19N04 and follow for approximately 1.5 miles. The forest service road 19N04 to access the site has both natural and gravel surface. The site is accessible year-round with a standard vehicle except for after heavy rains. C.3 MAGNITUDE OF CONSTRUCTION PROJECT FAR 32.204 - Disclosure of Magnitude of the Construction Project: o Between $100,000 and $250,000 Work Plan / Capability of the Offeror (NTE 2 pages): The offeror shall compose a narrative detailing a response to the below questions / statements. 1. Any information that indicates the extent to which the site for this project was visited. 2. The results concluded from the site visit. 3. The resources (personnel and equipment) planned for use on the project and their availability. a. Document any given concurrent obligations. b. Include any subcontractors and their experience and capabilities. 4. Site-specific safety plan. 5. Plan of work to include the major tasks and timelines. 6. Where applicable, estimated production rates. PACKING AND MARKING All shipments of materials, equipment and/or supplies shall be addressed to the Contractor and not to the Government. D-2 PROJECT LABELING FOR OFFICIAL COORESPONDENCE The Government singularly identifies each project with a contract number at time of award. The contract number is a unique identifier to purposely and permanently represent an awarded project. The Government issued contract number is to be referenced on all official communication starting upon notice of award. The Contractor shall be required to (a) commence work under this contract within 10 calendar days after the date the Contractor receives the notice to proceed, (b) prosecute the work diligently, and (c) complete the entire work ready for use not later than 30. The time stated for completion shall include final cleanup of the premises.
