In addition to supplying this form, each trade Contractor is also required to answer the following questions and provide these answers with your AIA Document A305: 1) List of trade union agreements and date the current agreement expires. 2) Within the last seven years, has any officer or principal of your organization ever been an officer or principal of another organization when it failed to complete a construction contract (if the answer is yes, please attach the details.) 3) On a separate sheet, list the major projects your organization has completed in the past five years giving the name of the project, owner, architect, contract amount, date of completion and percentage of the cost of the work performed with your own forces. 4) Trade/Supplier References (minimum of 3) The competency and responsibility of the bidders will be considered in making awards.
