THIS IS A SOURCES SOUGHT NOTICE FOR INFORMATION ONLY: THIS IS NOT A REQUEST FOR QUOTE Project: Main Entrance Enclosed Cover with Automatic Doors (GIMC Buildings: 2000 & 2017) Notice ID: GIMC-SS-FM-2022-001 The Indian Health Service, Gallup Service Unit, Gallup Indian Medical Center, located at 516 E Nizhoni Blvd, Gallup, NM, 87301, is seeking capable sources for the construction and installation of main entrance enclosed cover with automatic doors. Information received resulting from this sources sought notice will be used to determine acquisition strategy such as competition, set aside, etc. The work to be performed under this contract consists of providing all labor, material parts, and equipment necessary to construct two (2) main entrance enclosed covers with installation of automatic doors for Building 2000 and Building 2017. All project components and materials shall be installed in accordance with the manufacturer's technical installation instructions and specifications. All measurements and quantities are estimated, contractor is to confirm and adjust all measurements and quantities. Contractor shall provide all tools, qualified labor, materials, equipment, supplies and supervision necessary to engineer, install, and warranty all items in accordance with the following Statement of Work. All work shall be accomplished in accordance with all Federal, State, Local Regulations and the guidelines & requirements to include but not limited to the following: 1. American Society for Testing and Materials (ASTM) E2395-18 2. American Society for Testing and Materials (ASTM) F1196-19 3. American Society for Testing and Materials (ASTM) F842-17 The anticipated performance time is 60 calendar days. Dollar magnitude for this contract is between $100,000 and $250,000. The NAICS Code assigned to this acquisition is 238290 Other Building Equipment Contractors with a Size Standard of $16.5M. Firms responding must indicate their size in relation to this size standard, and indicate socioeconomic status (8(a), SDVOSB, HUBZone, EDWOSB, WOSB) or SB. If a response from two or more qualified firms is received for relevant socio-economic categories, this requirement may be set-aside. NOTE: If this requirement is set-aside, FAR 52.219-14 (DEVIATION 2019-01) Limitations on Subcontracting will apply; similarly-situated entity description applies. This clause requires that the concern perform at least 15 percent (15%) of the cost of the contract, not including the cost of materials, with its own employees.
