Construction of Handicap Concrete Ramps Per Ada Requirements, Construction Staking of Ramps, Detectable Warning Field, 5" Concrete Walks, Concrete Walk and/or Driveway, Concrete Curb and Gutter, 7" Concrete Pavement Variable Thickness Concrete Base, Variable Thickness Asphaltic Concrete Pavement, Various Concrete Removals, Sewer Work, Appurtenance Underground Work, Etc.; Located at: St216210111 - Ada Ramps for 2022 High Impact Paving @ Various Locations (North Side Projects) The scope of this contract is to remove and replace all of the pedestrian ramps within the limits of each of the City of Milwaukee "High Impact" asphalt paving projects. The "High Impact" asphalt paving projects are as follows: O.N. # 6-1-2022 North 56th Street - West Hampton Avenue to West Villard Avenue North 66th Street - West Port Avenue to West Bradley Road North 70th Street - West Congress Street to West Hampton Avenue North 80th Street - West Center Street to West Burleigh Street North 82nd Street - West Center Street to West Burleigh Street North 95th Street - West Silver Spring Drive to West Carmen Avenue North 104th Street - West Grantosa Drive to West Congress Street West Appleton Lane - West Florist Avenue to North 115th Street West Port Avenue - North 60th Street to North 66th Street O.N. # 7-1-2022 North 7th Street - North Doctor Martin Luther King Junior Drive to West Capitol Drive North 13th Street - A point south of West Vine Street to West Brown Street North 32nd Street - West Atkinson Avenue to West Cameron Avenue North 47th Street - West Vliet Street to West Lloyd Street North 51st Street - West Lisbon Avenue to West Center Street North Hopkins Street - West Villard Avenue to North Sherman Boulevard West Locust Street - West Appleton Avenue to West Lisbon Avenue West Melvina Street - North 46th Street to West Leon Trail Time for Completion: All work on this contract shall be completed by August 31, 2022. Liquidated Damages, per diem: $500.00/Day The residency requirement for this project is 40% of all hours worked on the project. One quarter of these hours must be attributable to residents living in the following zip codes: 53204, 53205, 53206, 53208, 53210, 53212, 53215, and 53233. The SBE requirement for this project is 25% of the contract base bid. The SBE Compliance Plan (Form A) must be submitted with the bid and is an element of bid responsiveness. Incomplete SBE Compliance Plans (Form A) or inaccurate SBE Compliance Plans will not be accepted and will render the bid unresponsive. The apprenticeship requirements for this project are: 0% . If this contract is awarded at $500,000 or higher, 25% of all apprentice labor hours must be worked by apprentices certified for participation in the Residents Preference Program and 40% of all apprentice labor hours must be attributed to apprentices living in the following zip codes: 53204, 53205, 53206, 53208, 53210, 53212, 53215, and 53233 The contractor shall specifically note the SBE, residency, and apprenticeship forms for this project. If the forms are not filled out properly, it will be cause for rejection of the bid. Bid Security Required: Bond, Certified Check, Cashier's Check or Cash to accompany bid: 10% of the Contractor's Base Bid. For those bids being submitted through Bid Express and using a manual Bond, the contractor may send a signed/executed copy of the bid bond to the Department of Public Works Contract Office by emailing DPWbids@milwaukee.gov. THE EMAILED COPY OF THE BOND MUST BE RECEIVED BY NO LATER THAN 10:30 A.M. ON THE SAME DATE THAT THE BIDS ARE DUE FOR THAT SPECIFIC OFFICIAL NOTICE, or the bid will be considered non-responsive. The original bid bond document must then be submitted by the contractor to the Department of Public Works Contract Office in-person or by mail to 841 N Broadway, Rm. 506, Milwaukee, WI 53202, immediately upon being notified that they are the apparent low bidder. IMPORTANT NOTICE: All bid documents for the listed project(s) are available electronically portal and can be submitted using either Bid Express or by emailing PDF copies of the complete bid documents to DPWbids@milwaukee.gov before 10:30 am on the date of the bid opening. However, Bidders are strongly encouraged to utilize All Contractors awarded a contract valued at $25,000.00 or more are required to participate in training on the City of Milwaukee's B2GNow contract compliance software within 30 days of the contract award date. Throughout the contract term, Contractors are required to regularly provide timely subcontractor payment information in the City's contract compliance software. The Prime Contractor awarded this project is required to participate in training on the City of Milwaukee's LCP Tracker Labor Compliance Software after the date of contract award. Throughout the contract term, Contractors are required to regularly provide timely payroll information via LCP Tracker.
