STATEMENT OF WORK RH Johnson Charleston VA Medical Center Padded Flooring, Walls, and Doors for ED Mental Health Seclusion Rooms GENERAL INFORMATION 1. Scope of Work: The Contractor shall provide all labor and materials necessary to accomplish the deliverables described in this statement of work (SOW), described under Specifications. The work includes supply and installation for padded with backing flooring, walls, and doors for two seclusion rooms in the Emergency Department Mental Health holding area at the Charleston VA Medical Center 109 Bee Street, Charleston, SC. Project is located on first floor. Space has one Norix bed bolted down in each room. Padding material shall be Gold Medal Safety Padding or equal. Period of Performance: The installation shall commence within (45 days) of award/review and the Contractor will provide the specified flooring with delivery and installation during normal business days, Monday through Friday and at normal hours of operations, (7:30AM through 4PM), or as agreed by the Contractor and the Government. Shall the Contractor propose an earlier completion date, and the Government accepts the Contractor's proposal, the Contractor's proposed completion shall prevail. The Contractor's work shall not take place at the Government site on Federal Holidays unless directed by the CO. Deliveries shall be made through employee and service entrances. Type of Contract: FIRM FIXED PRICE CONTRACT GENERAL REQUIREMENTS 1. The Contractor shall provide in writing a delivery and installation schedule. All written deliverables shall be phrased in layperson language. Statistical and other technical terminology shall not be used without providing a glossary of terms. 3. Where a written milestone deliverable is required in draft form, the VA will complete their review of the draft deliverable within 3 calendar days from the date of receipt. 1.1 SCOPE A. Provide all labor and materials required to furnish and install protective padding for the floors and walls in two seclusion rooms in the ED Mental Health identified as room numbers CC126 and CC127. B. Space has one single Norix bed bolted down in each room. C. Both room sizes are approx. 8'-5" x 9'-3" with a hard ceiling height of 8'-0". See drawings. SUBMITTALS A. Submit shop drawings showing list of materials, sections, thickness of materials and any other pertinent information. B. Submit three (1) samples - minimum of 3" x 3" in size - for approval and acceptance of protective padding system. C. Submit two (2) copies of the manufacturer's maintenance instructions. D. Submit two (2) copies of the onsite repair manual for any repairs that the facility may wish to undertake. E. Submit third party documentation showing compliance to ASTM Fire Testing detailed in 2.3 Properties. Testing must be for entire system, as opposed to individual components. All responses to questions shall be incorporated into a written amendment posted to the Government Point of Entry (GPE). The offeror is encouraged to monitor the GPE, www.sam.gov website with respect to this solicitation because any amendments to this Solicitation will be posted on the website (www.sam.gov). There will be no automated email notification of amendments. 9. Quotes must be received by March 04, 2022 by 2:00 PM ET. Email your quote to Jacob.tackett@va.gov. The subject line must have the solicitation the offeror is responding to listed. There will be no automated email notification of receipt of quotes. All quotes received without requested documentation will not be considered. 10. The offeror will submit one document in PDF form only, no word documents will be accepted.
