The City of Idaho Falls is preparing a call list to assist with waterline surface repairs concrete and asphalt replacement services (i.e. curb, gutter, rebar, sidewalk, driveway/approach and another related flatwork) for the 2022 season. Qualified vendors may submit a bid to provide outlines services for the City. Concrete work services are to be provided on an as needed basis, as determined and directed by the City of Idaho Falls. In order to submit a responsive bid, you must provide the following: Completes 2022 Waterline Surface Repair Quote (Attached) A current certificate of liability from your insurance company showing your coverage on all equipment. Proof of Worker s Compensation (Please see the attached copy of sub-sections 103.06 through 103.09 of the Standard Specifications - City of Idaho Falls, for minimum requirements). The enclosed proposal form listing rates for placement of specified concrete and/or asphalt by linear and square footage. Proof of a Public Right of Way Contractor s license issued by the City of Idaho Falls. Once submitted, your bid to provide concrete work services is firm for the season and not subject to change. Please include the name and telephone numbers of the initial contact person for your company on your bid.
