Forest Hills Public Schools, 6590 Cascade Road SE, Grand Rapids, Michigan, will receive proposals from qualified contractors for the 2022 Paving Projects at Northern High School, 3801 Leonard St NE, Grand Rapids, MI 49525 and Northern Hills Middle School, 3775 Leonard St NE, Grand Rapids, MI 49525 The Owner will enter into one contract with the successful General Contractor, who will be fully responsible for coordination of all required work between all trades All bids shall be accompanied by a sworn statement disclosing any familial relationship that exists between the owner(s) or any employee of the bidder and any member of the Board of Education of the School District or the Superintendent of the School District. The Board of Education shall not accept a bid that does not include a sworn and notarized familial relationship disclosure statement All bids shall be accompanied by Affidavit of Compliance - Iran Economic Sanctions Act in compliance with "Iran linked business" within the meaning of the Iran Economic Sanctions Act, Michigan Public Act No. 517 of 2012. The Board of Education shall not accept a bid that does not include the Affidavit of Compliance. Applicable Michigan use and sales tax apply to this project. All bids should be accompanied by a Background Check & Sex Offender Affidavit. The Board of Education shall not accept a bid that does not include a sworn and notarized background check and sex offender disclosure statement The Owner reserves the right to accept or reject any or all bids and to waive all irregularities in Proposals. Proposals shall remain firm for sixty (60) days from date of Bid Opening.
