Scope of Services The work shall consist of, but is not limited to, the contractor supplying the labor, equipment, superintendence, material, etc. to perform patching of asphaltic concrete roadway or parking lot failures, within the incorporated limits of Lafayette. The project will also include work to be done on an as needed, on-call basis. LCG will utilize a minimum of 16 tons or 30 square yards per call out. Removal and disposal of existing in-place material shall be the contractor's responsibility. The contract begins within ten (10) working days from the receipt of the "Notice to Proceed" and extends through May 31, 2023 with an option to renew June 1, 2023 through May 31, 2024. The Lafayette Consolidated Government reserves the option to increase contract quantities should additional funding become available. The contract may be renewed for a period of one (1) year beyond the primary term of this contract at the mutual agreement of owner and contractor and at the same price, terms and conditions. Contractors wishing to submit their bid electronically must first be registered online with Lafayette Consolidated Government as a potential supplier at the website listed above. Bidders submitting bids electronically are requested to provide the same documents as bidders submitting through the mail as soon as available. Only a bid bond, certified check or cashier's check shall be submitted as the bid security. Electronic copies of both the front and back of the check or bid bond shall be included with the electronic bid. Bids must be signed in accordance with LRS Title 382212(B)5. A Corporate Resolution or Certificate of Authority authorizing the person signing the bid is required to be submitted with bid. Contractors shall be licensed for the classification of "HIGHWAY, STREET AND BRIDGE CONSTRUCTION".
