Box 4030 Petersburg, VA 23803 Using Agency and/or Location Where Work Will Be Performed: Central State Hospital 26317 W. Washington St. Petersburg, VA 23803 Period of Contract: May 1, 2022 through two years 1.0 PURPOSE The intent and purpose of this Invitation for Bids (IFB) is to establish a contract with one (1) qualified source to furnish and set in place trash containers and provide refuse collection services for Central State Hospital (CSH) and Hiram W. Davis Medical Center (HWDMC), hereinafter referred to as the Agency. 2.0 BACKGROUND The Department of Behavioral Health and Developmental Services, Petersburg Campus is located on a 580 acre tract outside Petersburg, Virginia and consists of two Agencies of the Commonwealth of Virginia. The Campus houses the patient and residential populations of Central State Hospital, a psychiatric inpatient treatment facility and Hiram W. Davis Medical Center, an acute care facility. 3.0 SCOPE OF WORK: Contractor shall provide all labor, materials, supplies, transportation and equipment, except as noted herein, necessary to provide trash and garbage removal service. A. All garbage and refuse shall be collected and disposed of in an manner approved by the Commonwealth of Virginia, Department of Health and the Purchasing Agency. B. Contractor shall furnish and install 24 trash and garbage containers (6 & 8 cubic yard), one box dumpster, one 30-yard open top, and one 34-yard compactor with cart dumper, as specified on Attachment D. C. Contractor shall maintain all containers in good repair and appearance at all times. D. Painting/cleaning/odor control: Painting, cleaning and spraying for odor control may be scheduled at the contractor's option, with the understanding that specific problems reported by the Agency shall be corrected within one (1) business day of notification, either by on-site service or replacement at no additional cost to the Agency. E. Maintenance/Repair: It shall be the contractor's sole responsibility to institute a regular maintenance program to ensure continued serviceability of containers. However, any damaged containers reported by the Agency shall be repaired within one (1) business day of notification, either by on-site or replacement, at no additional cost to the Agency. F. Container Re-Positioning: Care shall be taken with regards to returning all containers to their original position with tops closed after emptying. G. Contractor shall provide 4 types of containers as described on Attachment D. H. Contractor shall be responsible for keeping the area around the containers free of garbage and refuse at time of pick-up. I. Contractor shall take reasonable precautions to prevent damage to roadways by equipment when accessing containers. J. The 34-yard compactor and cart dumper must be new or recently rebuilt with no hydraulic leaks and recently painted. K. Contractor shall empty the 34-yard compactor each Wednesday and clean (completely wash the inside of the container) L. Contractor shall empty or relocate the 30-yard container on request. M. Contractor shall empty 6-yard and 8-yard containers at all buildings a minimum of three (3) times per week (Monday, Wednesday and Friday). N. Additions/Deletions: The Agency reserves the right to add or delete containers as required. Additions and/or deletions will be accomplished by issuance of a Contract Modification, with resulting price changes in accordance with the firm fixed unit prices indicated in the Pricing Schedule. There may be other occasions when the Agency will need to temporarily rent a roll-off for various reasons and for various lengths of time. Temporary rentals will be priced separately. O. Disruption of Service: Should inclement weather conditions interrupt or delay services, the contractor shall advise the Agency Representative of the stoppage or delay and alert them as to the alternate pick-up day be 8:30 AM of the affected day. P. Contractor's Facilities/Equipment: The Contractor shall have sufficient manpower and equipment available to ensure that satisfactory services is provided to an account of this magnitude and diversity, including the necessary facilities and technical skills needed to ensure continuous, uninterrupted service during inclement weather. A contractor's inability to start and/or operate equipment will not be an acceptable reason for non-performance of any service described herein during inclement weather or otherwise. Q. Contractor's Employees: The Contractor shall provide competent drivers whom possess a valid Commercial Driver's License. All contractor personnel shall wear an appropriate uniform at all times while servicing this account. At a minimum , the uniform shall clearly indicate the employee's name and have some type of insignia indicating the company's name. The Agency reserves the right to bar any driver from Agency premises if the driver demonstrates any type of safety threat to individuals and/or staff, operates any vehicle in a manner causing damage or potential damage to buildings/structures and/or lawns, or demonstrates any other inappropriate behavior. R. Missed Pick-Ups: The Agency will notify the contractor on any containers missed during scheduled service days. The contractor shall have those containers picked up the day of notification. Vehicles or other impediments to timely pick-ups shall be reported immediately to the Physical Plant Services Office at 804- 524-7623. The contractor shall allow fifteen (15) minutes for Agency personnel to move the impeding before by-passing the container. In this case, the contractor would not be responsible for emptying the container until the next scheduled visit. The contractor shall be responsible for same day pick-up of all containers missed on service days in which he/she fails to complete the service within the scheduled hours of operation.
