THE UTILITIES BOARD OF THE CITY OF BAYOU LA BATRE NORTH SYSTEM, PHASE 1 Consisting of approximately 7,900 feet of 4" HDPE sewer force main, 17,280 feet of 2" HDPE sewer force main, 104 grinder pump service connections, and all other necessary appurtenances as detailed in the design plans. The Information for Bidders, Form of Bid, Form of Contract, Plans, Specifications, and Form of Bid Bond, Performance and Payment Bond, and other Contract Documents may be examined at Speaks & Associates, Consulting Engineers, Inc., 732 Oak Circle Drive West, Mobile, AL 36609. The Request for Bid Documents form may be obtained by emailing brendah@speaks.cc and placing the project name in the subject line. The Owner reserves the right to waive any informalities, or to reject any or all bids, and to award the contract to the lowest responsible, responsive bidder. All bidders must comply with the requirements of the Contractor's Licensing Law of the State of Alabama and be certified for the type of work on which the proposal is submitted. Each bidder must deposit with his bid, security in the amount, form and subject to the conditions provided in the Information for Bidders. The Utilities Board of the City of Bayou La Batre is a drug-free workplace. All contractors that bid or work on The Utilities Board of the City of Bayou La Batre projects must agree to adhere to The Utilities Board of the City of Bayou La Batre's drug-free workplace policy regardless of number of employees. This applies to contractors and all levels of subcontractors. Copies of this policy are available upon request. The Contractor shall provide with his bid a "Qualification Letter" on company letterhead with the following information: 1. Contractor's Years in Service for the submitting entity including the date of incorporation, and that it is otherwise a licensed and valid corporation under the laws of the State of Alabama. 2. Statement that the Contractor submitting the bid has no less than two calendar years of prior experience that includes projects with the same type and size of this project that were completed successfully. 3. A list of any projects that the Contractor has defaulted within the last five years. 4. A list of projects that required involvement or notification to the Contractor's surety within the last five years. 5. A list of any lawsuits filed against the Contractor or Principal Owner of the Contractor involving non-payment of monies due to a lending agency or supplier, subcontractor or by the surety within the last five years. 6. Statements that the Contractor has in its employment, qualified experienced employees that have successfully performed the same or similar type of work. Following the bid opening, the Owner may request additional documentation substantiating the representations made in the "Qualification Letter". No Bidder may withdraw their bid within ninety (90) days after the date of the opening thereof. All bids must be submitted in a sealed envelope bearing on the outside the name of the Bidder, Bidder's License Number, address, and name of the project.
