Epoxy Floor Repair Service Base Year Ffp Epoxy Floor Repairs / Depot Wide Epoxy Floor Repairs - Base Year Contract Shall Consist All Labor, Materials, Appliances, Transportation and Supervision Required to Perform All Necessary Operations in Connection With the Repair of Epoxy Floors as Needed or Requested by the Government. The Price Will Be Calculated Based On the Aggregate Total of All Fields Located in the Bid Schedule of the Attached Statment of Work. This Bid Schedule in Section 9 of the Attached Statement of Work Must Be Completed for Bids to Be Considered. Must Bid on All Items to Be Considered. **note: (Note to Offerors: Offeror Must Provide With Their Bid, Any Msds and Product Data Sheets for the Materials That Will Be Used. These Must Be Provided With Offers to Make Sure the Materials Are the Same Form/fit/function as Those Being Requested in This Statement of Work. These Documents Must Be Provided With the Offer in Order for the Offer to Be Considered. The Government Is Under No Obligation to Use All or Any of the Funds Provided For This Contract. No Repairs Shall Be Made Until the Vendor Is Notified to Proceed by Contracting. The Award Amount of This Contract Is a Ceiling for the Work to Be Performed. Payments Will Only Be Made for the Actual Work Performed. The Contractor Exceeds the Ceiling at Their Own Risk. Changes to the Contract Ceiling Will Be Made by Contract Modification Only. Please Use Solicitation Number 22q0079 in Subject Line When Submitting Quote via Email to: Maria.t.lucarine.civ@mail.mil Period of Performance Dates Are Estimated - Actual Dates Will Be Determined at Time of Award GENERAL: This will consist of repair and maintenance services associated with the "Epoxy Floor Repairs". The repair services necessary to complete this includes providing all licenses, permits, labor, materials, equipment, supervision and management to complete the project. All and any exceptions shall be submitted in writing. 1.1. Description of Services/Introduction: Epoxy floor repairs shall consist of no less than 250 sq. ft. per service call. Pictures (when available) and approximant Square footage will be provided to the contractor prior to scheduled work. The epoxy flooring system shall be installed in accordance with the manufacturer's recommendations and the specifications herein. Removal of all construction/demolition debris shall be the sole responsibility of the contractor. The contractor shall dispose of any waste materials according to Tobyhanna Army Depot Recycling Policy. 1.2. Objectives: Basic objectives of this service contract are to repair existing flooring that has created tripping hazards or has general failures, including but not limited to 1/4 " thick resinous flooring, fillers, concrete, etc. 1.3. Scope: The contractor shall provide repairs for epoxy flooring. 1.4. Period of Performance: The period of performance shall be for one (1) Base Year of 12 months and two (2) 12-month option years. These option years shall be exercised at the convenience of the government, and are not guaranteed. 1.5. General Information 1.5.1. Quality Assurance: The government shall evaluate the contractor's performance under this contract in accordance with the Quality Assurance Surveillance Plan. This plan is primarily focused on what the Government must do to ensure that the contractor has performed in accordance with the performance standards. It defines how the performance standards will be applied, the frequency of surveillance, and the minimum acceptable defect rate(s). 1.5.2. Quality Control: Quality control shall be the responsibility of the contractor. The contractor shall meet all federal and Tobyhanna Army Depot safety standards at all times. The contractor shall meet all U.S. Army and Federal quality and workmanship standards. The contractor shall install/repair all epoxy according to manufacturer specifications. 1.5.3. Rejection of work: Indication of lack of quality workmanship on the part of the contractor shall be sufficient grounds for the government to reject installed work and to require its immediate removal and complete reinstallation at no additional cost to the government. 1.5.5. Hours of Operation: Normal business hours are between the hours of 0730 and 1600 Monday thru Friday except Federal holidays or when the Government facility is closed due to local or national emergencies, administrative closings, or similar Government directed facility closings. 1.5.6. Contractor Operations: Contractor may be required to perform work on off hours and/or weekends. The Contractor must at all times maintain an adequate workforce for the uninterrupted performance of all tasks defined within this PWS when the Government facility is not closed for the above reasons. When hiring personnel, the Contractor shall keep in mind that the stability and continuity of the workforce are essential. 1.5.6.1. Historically, 75% of all epoxy repairs have been performed after 1630 Monday through Friday and during the weekends. 1.6. Coordination: All work shall be scheduled so as to provide minimum interference with Tobyhanna Army Depot operations. The contractor shall coordinate all scheduled work with the POC prior to the commencement of work and at the convenience of the government. 1.7. Place of Performance: The work to be performed under this contract will be performed at governmentowned facilities of Tobyhanna Army Depot. 1.8. Type of Contract: The government will award a firm fixed price contract (FFP).
