The City of Danbury Department of Health and Human Services is seeking the services of a qualified individual or firm to provide on-call consulting services on an as needed basis for work related to Environmental Impact Commission (EIC) application reviews. The City of Danbury Department of Health and Human Services is seeking the services of a qualified individual or firm to provide on-call consulting services on an as needed basis for work related to Environmental Impact Commission (EIC) application reviews. The selected individual or firm will be required to attend EIC meetings. The consultant may be requested to provide services on short notice and within a tight time schedule. The contract will be for three (3) years with the possibility of a mutually agreed upon two (2) year time extension. No guarantee is made as to the quantity of work to be awarded related to this on-call services contract. The selected individual or firm will be required to provide copies of their professional liability and general liability insurance certificates naming the City as an additional insured which insurance certificates are to be in forms acceptable to the City's Risk Manager. The types and limits of required insurance coverage are indicated in the sample City/Consultant contract attached hereto. Modifications to the sample contract may be made for this specific project.
