Scope: Leffingwell Road Culvert Replacement MAH-C.R. 74-12.38 Replacement of two (2) 65" span x 40" rise arch corrugated metal pipes with one (1) 9'-6" span x 6' rise precast concrete box culvert and other related work. Checks are to be made payable to Treasurer of Mahoning County. Failure to obtain a copy of the plans and specifications from the Mahoning County Purchasing Department may result in bid rejection/s due to non-compliance with fee requirements and official addendum notification/s. Bids of Corporations not chartered in Ohio MUST be accompanied by proper certifications that such Corporation is licensed to do business in Ohio. Attention of the Bidder is directed to the requirements that each proposal must be accompanied by a non-collusion affidavit, properly executed by the Bidder. Further, the successful Bidder will be required to execute an affidavit required by Sec. 5719.042 of the Ohio Revised Code. No payment shall be made on any contract for which no such affidavit has been submitted. All Contractors must be pre-qualified with the Ohio Department of Transportation. Domestic Steel use requirements as specified in Section 153.011 of the Revised Code apply to this project. Copies of Section 153.011 of the revised Code can be obtained from any of the offices of the Department of Administrative Services. All steel products must meet the requirements of ODOT CMS 106.09. The contractor and all subcontractors for this project must be enrolled in the Ohio Bureau of Workers' Compensation Drug Free Workplace Program or in a similar program approved by the Bureau of Workers' Compensation. Any contractor that does not meet this requirement will have their bid deemed non-responsive and cannot be awarded the contract. The Board of Mahoning County Commissioners reserves the right to reject any or all bids and to waive informalities. In addition, the Board of Mahoning County Commissioners reserves the right to participate in state contracts which the Department of Administrative Services, Office of State Purchasing has entered into for the purchase of supplies, services, equipment and certain materials pursuant to Ohio Revised Code Section 125.04. No bids may be withdrawn for at least sixty (60) days after the opening thereof. Each bidder must insure that all employees and applicants for employment are not discriminated against because of race, color, sex or national origin.
