This Request for Quotation (RFQ) is being issued by the Milwaukee Public Schools (MPS) Division of Facilities and Maintenance Services (DFMS) to furnish all labor, tools, equipment and materials necessary plant trees at Custer Playfield, per the following summary, specifications and drawings/photos: PERFORMANCE AND PAYMENT BONDS: 2.1 All Quotes in excess of $10,000 must include the cost of a one hundred (100%) percent Performance and Payment Bond as security for the completion of all work and for the payment of all persons performing labor and furnishing materials. The successful bidder will receive the appropriate bond forms at the time of issuance of a Service Order to proceed, and the completed forms must be returned within ten (10) days of receipt. No work may proceed until the completed bond forms are returned and failure to submit the bonds will result in non-payment. RFQ Question Deadline: No questions will be answered verbally and no verbal information from any source is to be relied upon by any respondent in the development of their response to this RFP. Only questions submitted prior to 12:00 PM, Noon on Wednesday, April 06, 2022 will be answered. No questions will be answered after that time and date. The responses will be documented by way of addenda, which will be forwarded to all firms who have requested information about this RFQ. COMPLETION DATE: 3.1 Work on this project shall be commenced on site on April 12, 2022, or at such date thereafter as may be required by conditions or as directed by the Director of the Division of Facilities and Maintenance Services, and all work shall be completed by: April 29, 2022 3.2 The time of completion shall be STRICTLY ADHERED TO. In case of failure on the part of the Contractor to execute his work satisfactorily, the Board reserves the right to employ other means to complete the work AND In case of failure to complete the work within the time provided as stated herein the Quotation Form, MPS reserves the right to assess liquidated damages, in the amount of $50 per calendar day.
