A copy of the Bidder's Business Registration Certificate issued by the NJ Department of Treasury may be provided with the bid proposal at the time of bid opening. If awarded, vendors shall submit a BRC prior to the time of contract award or authorization . This requirement is mandatory and failure to comply will disqualify the bid proposal. -Bidders shall comply with the required completion and submittal of a Non-Collusion Affidavit. -Bidders are required to comply with the requirements of N.J.S.A. 10:5-31 et seq. and N.J.A.C. 17:27 -One of the following documents will be accepted: Letter of Federal Approval, Certificate of Employee Information Report or complete AA302/AA201 form. -As evidence of good faith, a BID BOND shall accompany each bid/proposal or CERTIFIED CHECK made payable to PATERSON PUBLIC SCHOOLS in the amount of 10% of total bid amount, not to exceed $20,000.00 dollars. - If required, Bidder must submit with his/her bid a certificate from a surety company stating that the surety company will provide the Contractor with a performance bond in an amount equal to the amount of the contract (N.J.S.A. 18A:18A-25). Such surety company must be licensed and qualified to do business in the State of New Jersey. The certificate (Consent) of Surety, together with the power of attorney must be submitted with the bid. Failure to submit the certificate (Consent) of Surety shall be cause for disqualification and rejection of bid. - If required, Bidder shall comply with the State of New Jersey Public Works Contractor Registration Act, if applicable. A copy of the registration application form or registration certification must accompany all bids. - Each Bidder shall provide certification indicating he/she owns, leases, or controls all the necessary equipment required by the specifications to perform the contract. - If applicable, Bidder shall comply with the State of New Jersey Prevailing Wage Act for public works, if applicable, and will be required to show proof of prevailing wages to any and all employees involved in the performance of this contract. Bidders are encouraged to contact the New Jersey Department of Labor, Prevailing Wage Rate Determination Office and request current copies of the Passaic County wage and benefits rates. The successful Bidder (Contractor) shall submit certified payrolls for public works, if applicable, to the Director of Facilities to obtain payment under the awarded contract. -Bidders shall submit with their bid: name, address, and telephone number of three (3) references -The Bidder is advised of the responsibility to file an annual disclosure statement on political contributions with the New Jersey Election Law Enforcement Commission pursuant to N.J.S.A. 19:44A-20.27 (P.L. 2005, c271, s.3) The District reserves the right to reject all bids, to waive minor informalities or non-material exceptions of any bid or bids and to make such awards to the lowest responsible Bidder in accordance with Public School Contracts Law.
