INTRODUCTION This project consists of providing all materials, equipment and vehicles, labor, tools, parts, transportation and incidentals to provide new asphalt pavement of the North Shore - Lynx Lake Store Road (FSR 611): Station 0+00 to Station 10+79. This project consists of doing a Stabilized Full Depth Reclamation (SFDR) with Portland Cement up to a depth of 8"-10" and compact in-place. Install 2" of new Fiber Reinforced Asphaltic Concrete with PG 64-22 binder. Build-up low pavement shoulder areas where needed and Install asphalt curbing in areas where indicated. Install two asphalt aprons w/ curbing and Restripe Lynx Lake Store Road from Station 0+00 to Station 10+79. MAGNITUDE OF CONSTRUCTION PROJECT The Magnitude of the project is between $100,000 and $250,000. All work shall be in accordance with the specifications contained herein. No changes to the specifications, terms or conditions of this Request for Quotations shall be made without the prior approval of the Contracting Officer in writing. 2.02 BACKGROUND The existing asphalt road is deteriorating showing alligator cracks and ruts through out. This area serves as a recreation site to the general public and receives constant and regular use and needs to be well maintained. 2.03 PROJECT LOCATION Lynx Lake North Shore is located within Prescott National Forest, approximately 5 miles east of Prescott, Arizona, in the Bradshaw Mountains. It is one of the most popular recreation areas in central Arizona. From Hwy 69 Head South on Walker Road to North Shore Lynx Lake Store Road to project site. Project starts where Walker Road and Lynx Lake Store Road join and continues East .21 Miles (Sta. 0+00 to Sta. 10+79). For project boundary refer to Section 6 Exhibit A. 2.04 PROJECT REQUIREMENTS The scope of this project consists of providing all materials, equipment and vehicles, labor, tools, parts, transportation and incidentals for the new Stabilized Full Depth Reclamation (SFDR) with Portland Cement of the North Shore Lynx Lake Store Road (FSR 611), stabilization of shoulder build-up, curbing and restriping. All work shall be completed in accordance to specifications. FP-14 Specifications have been modified/clarified by FS. 2.05 GENERAL SITE CONDITIONS, FACILITIES AND TEMPORARY CONTROLS A. This site is located within the municipality of Prescott and is easily accessed via paved surface roads. B. Contracting Officer's Representative (COR) - The COR is a Forest Service official designated by the Contracting Officer to perform day-to-day contract administration, including site inspections for quality assurance. C. A site can be made available to the Contractor for staging and storage during the duration of the project and will be located by the COR at the beginning of work. Any damage resulting from the Contractor's use of this site will be repaired or replace at the Contractor's expense at the direction of the COR. D. Potable water is not available. All temporary services and use areas shall be completely removed and the use areas shall be restored upon completion of use. E. Transportation to and from the work site for the Contractor's personnel and materials will be provided at the Contractor's expense. F. The Contractor shall take reasonable measure to secure equipment and contract work against theft and vandalism. The Forest Service is not responsible for the theft and/or vandalism of any materials or equipment left at the project site. G. The work zones shall be delineated and marked adequately warn visitors, government contractors and all government employees of equipment operations and other hazardous conditions. Any damage to existing improvements shall be repaired at the Contractor's expense according to the direction of the COR. H. No Government furnished supplies will be provided. Unless instructed otherwise. I. Posting of any and all construction signs will be the contractor's responsibility. Work will be performed during business hours. J. Any disruption to the area must be pre-approved and coordinated with COR. K. A pre bid meeting will be set no less than five days prior to bids being due. L. Selected contractor will be responsible for Blue Staking the project area if needed. M. Selected contractor will be responsible for providing traffic control signage and trained traffic control personnel for this project. N. Contractor will be responsible for cleaning up daily and securing project area. *The value for this project is based on a financial range. The value is listed as the highest possible cost from the range provided by a stakeholder or official project document.
