All information shall be shown in the spaces provided including the signature of the Tenderer with their address and telephone number. Tender Forms must be properly signed and witnessed or signed and sealed if the Tenderer is a Corporation. Any Tender Offer sent by fax will be disqualified. The Tender must be legible, written in ink or typewritten, where stipulated, with the unit price for every item clearly shown. Tenders which are incomplete, conditional or obscure, or which contain erasures or alterations not properly initialled, or irregularities of any kind, may be rejected. Submissions must not be restricted by a statement added to the Township form or by a covering letter, or by alterations to the form supplied, unless otherwise provided in the Special Provisions. The contract documents shall consist of all pages of the Tender or Quotation documents issued by the Township, and the Contractor's submission. Do not remove any pages from the Township form(s). A certified copy of a comprehensive policy of: Public liability and property damage insurance providing coverage in respect of any one accident of at least two million dollars ($2,000,000) exclusive of interest and cost, against loss or damage resulting from bodily injury to, or death of, one or more persons, and loss of or damage to property Automobile liability insurance coverage of not less than two million dollars ($2,000,000) inclusive per occurrence. Such policies shall name the Township of Augusta as additional insured parties thereunder and shall protect the Township and its employees and servants as an additional insured for all damage or injury including death to any person or persons and for damage to any property of the Township or any other public or private property resulting from or arising out of any act or omission on the part of the contractor or any of his servants or agents. The policy shall be in effect during the execution of the work and for the associated warranty periods if required.A certificate of insurance shall be submitted upon award of the contract and yearly during the contract term.
