Prospective Bidders shall deliver completed City of Glendale Qualification Policy Form and required supporting information regarding their qualifications no lessthan five Business Days prior to the bid opening or by April 20, 2022, whichever is earlier One hard copy of the form and attachments can be submitted to the City of Glendale 5909 N. Milwaukee River Parkway, Glendale, WI 53209 (Attn: City Clerk) and marked with "Contractor Qualifications" on the\ envelope OR electronically submitted to Andrew Ashley at andrew.ashley@clarkdietz.com. The project consists of directionally drilling approximately 100 LF of 8" watermain in Good Hope Rd underneath a proposed culvert for Brown Deer Creek. The project will be completed in conjunction with Milwaukee County's Good Hope Rd. Roadway project.
