Roof Replacement Hocking Valley Community Hospital is a county-owned hospital organized under Chapter 339 of the Ohio Revised Code that serves Hocking County and the surrounding counties. HVCH is currently designated as a Critical Access Hospital . Pursuant to Sections 307.86, 307.87, and 339.05 of the ORC, HVCH is releasing this request for proposal for roof replacement of 601 State Route 664 North, Logan, OH 43138, consisting of approximately 93,000 square. HVCH is seeking skilled, competent completion on a commercially reasonable timeline. Our budget is approximately: $900,000 HVCH is unable to sign contracts containing indemnification and all contract must allow for assignment by HVCH. Governing law must be Ohio and venue must be solely in Hocking County. The below criteria have equal weight. The response to this RFP should include the following information at a minimum: 1. Name of your group including address, the primary contact person, telephone and fax numbers, and email address. 2. Description of your executive management team and organizational structure. 3. Detailed description of the ownership including all related entity(s) or contractual relationships relevant to your proposal. 4. Specify your current insurance coverage, including limits. 5. Provide a detailed overview of construction schedule and plan, with pricing, including, but not limited to removal and replacement of existing roof. 6. Describe any other "value added" services your group may offer. 7. Provide a list of references.
