patch mix, F.O.B. plant.
2.Furnish approximately
2,000 tons INDOT 402
type B surface intermediate and base.
3.Furnish approximately
15,000 tons coarse aggregate, F.O.B. plant.
4.Furnish approximately5,000
tons coarse aggregate, delivered.
5.Furnish approximately15,000
tons fine aggregate, F.O.B. plant.
6.Furnish approximately5,000
tons fine aggregate, delivered.
7.Furnish approximately1,000
tons sand, F.O.B.
plant.
8.Furnish approximately1,000
tons sand, delivered.
11.Awarded contract may
be extended for a
one-year periods by mutual agreements of the
parties.
Unless otherwise specified
by the City Engineer or
stated in Special Conditions, each item shall be
governed by the Indiana
Department of Transportation (INDOT) Standard
Specifications including
Recurring Special Provisions and/or Supplemental Specifications.
Each bidder must file
State Form 95 (as preLEGALS
scribed by the State
Board of Accounts).
Each bidder must file
with his bid the usual
statutory non-collusion
affidavit.
Bids will be evaluated on any and all factors deemed relevant by
the Board of Works and
Safety. The Board of
Works and Safety reserves the right to reject
all bids and to waive any
informalities or irregularities in any bid or bid procedure, and to accept
any bid that meets or exceeds the specifications
that is the lowest responsible and responsive bid
which shall be determined in the sole discretion of the Board of
Works and Safety.
All contractors, subcontractors and bidders for
contracts with the City of
Columbus shall submit a
written declaration specifically titled affirmative
action signature form
stating that they will not
discriminate against any
employee or applicant
for employment in the
performance of such
contracts with respect to
hire, tenure, terms conditions or privileges of employment or any matter
indirectly related to employment because of
race religion, color, sex,
disability, national origin,
or ancestry.
The declaration shall also
state that the contractor,
subcontractor and bidder
shall comply with this
statement as well as
these contractual promises. Failure to submit
this written declaration titled "affirmative action
signature form" will constitute a material defect
in the bids, and a breach
of this covenant may be
considered a material
breach of the contract.
