NOTICE TO BIDDERS VILLAGE OF McCOOK JOLIET AVENUE RESURFACING MFT SECTION NO.: 23-00000-01-GM TIME and PLACE OF OPEN ING OF BIDS: Sealed Proposals for the improvement described below will be received at the office of the Village Clerk, Village of McCook, 5000 Gelncoe Avenue, McCook, IL 60525, until 10 a.m. on Sentemher 12. 2023 and will be opened and read at that time. DESCRIPTION OF WORK: HMA surface removal; curb and gutter removal and replacement; sidewalk removal and replacement; structure adjustments; HMA surface course replacement; topsoil and sod: and all appurtenant construction at various locations throughout the Village of McCook B1DDF.RS INSTRUCTIONS: 1. Plans and proposal forms are available for download only from QuestCDN via the Novotny Engineering website, http://novotnyengineering.com, "Bidding" tab, for a non-refundable charge of $40.00. Please contact Novotny Engineering (630-887-8640) to obtain the QuestCDN password. 2.Prequalification If checked, the 2 low bidders must file within 24 hours after the letting an "Affidavit of Availability" (Form BC 57), in duplicate, showing all uncompleted contracts awarded to them and all low bids pending award for Federal, State, County, Municipal and private work. One original shall be filed with the Awarding Authority and one original with the IDOT District Office. 3. The Awarding Authority reserves the right to waive technicalities and to reject any or all proposals as provided in BLRS Special Provision for Bidding Requirements and Conditions for Contract Proposals. 4. The following BLR Forms shall be returned by the bidder to the Awarding Authority: a. BLR 12200: Local Public Agency Formal Contract Proposal b. BLR 12200a Schedule of Prices c. BLR 12230: Proposal Bid Bond d. BLR 12325: Apprenticeship or Training Program Certification e. BLR 12326: Affidavit of Illinois Business Office 5.
