The City of Warren, Ohio will accept bids for: TRU Reserve Avenue Bridges Project The contracts work includes furnishing all materials, equipment, tools, labor and services necessary for completion of the project. All work shall be done as shown on drawings called for in the description of the work and in a manner directed by the specifications of the City of Warren, Trumbull County, Ohio. The work to be performed under this project is located in the City of Warren, Trumbull County, Ohio. The TRU Reserve Avenue Bridges Project includes milling and resurfacing pavement, replacement of guardrail, removal of existing asphalt deck overlay and replacement with a concrete deck overlay, pavement markings, new crushed aggregate slope protection at two abutments, refurbishing bridge bearings, new vandal protection fence, concrete patching of curbs and substructure concrete, sealing of concrete surfaces, and other minor work. The Disadvantaged Business Enterprise goal for this project shall be 10%. NOTICE: See Section 7, ODOT LPA Template, Page 17, Item 25, LPA PN 013 5/26/2016 DISADVANTAGED BUSINESS ENTERPRISE (DBE) UTILIZATION PLAN AND GOOD FAITH EFFORTS, DBE UTILIZATION PLAN and DBE AFFIRMATION. All Bidders shall submit a DBE Utilization Plan before the time of bid opening to the following website. The apparent low contract must complete all the requirements set forth within the DBE AFFIRMATION within five days of the Bid Opening date. See the following website. ALL STEEL PRODUCTS MUST MEET THE REQUIREMENTS OF ODOT CMS 106.09 Prequalification Bidders must apply for pre-qualification with the Ohio Department of Transportation's (ODOT) Office of Contracts, Contractor Qualifications Section, at least 30 days before the date set to open bids. Only those contractors which are then currently pre-qualified by ODOT will be eligible to submit bids for the construction of projects addressed by this policy. The Board of Control reserves the right to reject any or all bids and to waive any informalities in bidding. Any bid guaranty shall be subject to the applicable provisions of Chapter 153 of the Ohio Revised Code to the same extent as if said provisions of Chapter 153 were fully incorporated within said bid guaranty. If the bid is accepted, the successful bidder shall enter into a contract and furnish a Contract Performance Bond as provided for below: Bids of corporations not chartered in Ohio must be accompanied by proper certification that such corporation is authorized to do business in Ohio. The successful low Bidder will be required to comply with Section 107.12 of the ODOT CMS which establishes minimum limits for insurance. Attention of bidders is directed to the Labor Provisions: This project is subject to the Wage and Hour Division of the U.S. Department of Labor. Minimum wages governing prevailing wage rates to be paid to the laborers and mechanics employed on public improvements. This Contract is subject to the provisions of Executive Order 11246 of September 24, 1965 as pertains to an Affirmative Action Plan, and Governor Executive Order of January 27, 1972. Each bidder must ensure that all employees and applicants for employment are not discriminated against because of race, color, religion, sex or national origin. The successful low Bidder will be required to comply with all contract requirements for Equal Employment Opportunities. All Bidders must fill in all the blanks of the proposal in ink or be typewritten. This agreement is subject to the requirements of the Clean Air Act, as amended, 42 USC 1857 et seg., the Federal Water Pollution Control Act, as amended, 33 USC 1251 et seg., and the regulations of the Environmental Protection Agency with respect thereto, at 40 CFR Part 15, as amended from time to time. No Bids may be withdrawn after the scheduled closing time for receipt of bids for at least sixty (60) days. The City of Warren will award this project within 90 days after bid opening. Engineer's Construction Cost Estimate: $ 1,404,033
