Introduction and Scope of ServicesThe Town of Hurley, NY (Town) is soliciting proposals from one or more qualified professional firms to provide mold remediation services for the Town. The Town may determine to appoint one or more firms for specific projects or other specified scope.The scope of services to be provided may include, but is not necessarily limited to, professional services for the following:Removal of any/all mold covered materials found present in the Town of Hurley highway garage. Clean all areas where mold is observed in the Town of Hurley highway garage and attached additions. Treat all affected areas with a non-toxic antimicrobial agent to inhibit regrowth of mold. Submittal of any further recommendations to prevent mold growth in the Town of Hurley highway garage. General Information: Include information the proposer deems relevant to its qualifications, expertise and capacity to provide timely services to the Town.Remediation Plan: Include information related to the treatment of affected areas and further recommendations to prevent mold growth in the Town of Hurley Highway Garage.Non-Collusive Bidding Certification Form: Submit the signed and notarized non-collusive bidding form with the proposal.Contract Price Proposal: A proposal shall be submitted to the Town of Hurley Supervisor, 10 Wamsley Place, Hurley, NY 12443 that includes the following:Hourly Rate Schedule - Includes schedule of hourly rates by staff/position level.Unit Cost for Disbursements - Include a schedule of disbursements as permitted by the form of contractOverall price estimateProcess for Questions and ClarificationsAny questions or requests for clarification with regard to this Request for Proposal should be submitted in writing to:Melinda McKnight, Supervisor Town of Hurley10 Wamsley PlaceHurley, NY 12443 supervisor@townofhurley.orgQuestions or requests for clarifications may be transmitted to the Town by US Mail, courier, e-mail or electronic facsimile. Confidential, trade secret or proprietary materials as defined by the laws of the State of New York must be clearly marked and identified as such upon submission. Proposers intending to seek an exemption from the Freedom of Information Law must request the exemption in writing at the time of submission, setting forth the reason for the claimed exemption. Any determination on the exemption request will be made in accordance with the Freedom of Information Law in the discretion of the Town.Ethics, Disclosure of Interests and Representations:The Town's Ethics Code is posted on the Town's website.Officers and employees are prohibited from entering into certain contracts with the Town pursuant to Article 18 of the General Municipal Law. Except as expressly provided in 802 of Article18 of the New York State General Municipal Law, no Town officer or employee shall have an interest in a contract that is prohibited by 801 of the General Municipal Law. Any contract willfully entered into by or with the Town in which there is an interest prohibited by that section shall be null, void, and wholly unenforceable to the extent provided by 804 of the General Municipal Law. The method of Contractor Selection has not been Determined at this time.
