Furnish all labor, materials, supervision, and equipment necessary to remove approximately 14,240 SF of concrete, roughly 4" to 8" deep and dispose of the existing concrete, excavate, grade and compact existing subgrade, install, grade and compact new crushed concrete subbase, install new asphalt surface and pavement markings. All work is limited to the concrete parking lot area only behind the north kennel building parking areas as illustrated on the attached Sheet No. 2 - Project Site Plan and Sheet No. 3 - Typical Light Asphalt Details as illustrated on the attached Project Site Plan Drawings. Qualification Requirements: 1. Contractor must be licensed in the State of Florida Certified General Contractor or Building Contractor and should provide proof of current license(s) with their quote submittal. 2. Contractor must have been in business under the same name or EIN number for a minimum of three (3) years. 3. Contractor must possess a Polk County Local Business Tax Receipt (f/k/a Business License) in order to do business with the County. A copy of such license must be provided to the Procurement Division before award is made to the successful bidder. 4. Provide a minimum of (3) three references for similar work successfully completed within the past two (2) years. Referenced work must be comparable in size and scope of work detailed in this solicitation. A detailed summary of work and completion dates must be provided.
