1. A Proposal Bond Approved by the County Solicitor in an Amount Equal to 10% of Total Bid, but Not in Excess of $20,000.00.
2. A Certificate (Consent of Surety) as Per N.j.s.a. 40a:11-22 From a Surety Company Stating That It Will Provide the Bidder if Successful, With a Performance Bond and Payment Bond in the Sum of Not Less Than the Total Contract.
3. A Completed Non-collusion Affidavit.
4. A Completed Public Disclosure Information Sheet Indicating Stockholders in the Corporation or Partnership Who Owns 10% or More of Its Stock as Provided in Chapter 33 of the Public Laws of 1977.
