Performance Period: 540 Continuous Calendar Days (See Page CC-3 and Appendix B) Liquidated Damages: Fifteen Hundred ($1,500) Dollars per Continuous Calendar Day and as set forth in the Construction Contract Page CC-3 and Appendix B he project is located at 99 MacFarland Avenue, Staten Island, NY 10305 Although this solicitation has been publicly, advertised only Contractors that are prequalified with the SCA are eligible to submit bids for this project. In your email bid, please send only bid page (appendix a), a scanned image of your bid bond, and wsf form (request for wicks - meps) if applicable. Please put the name of the school and borough and the solicitation number in the subject line and save each attachment with the name of the school and borough and the solicitation number in the document name. See specific Contractor-furnished insurance requirements and SCA-furnished Insurance Coverage, Builder's Risk Coverage and Deductibles, the Owner Controlled Insurance Program (OCIP). The OCIP Manual is available at the following portal. A Bid Bond prepared on the form attached to the Construction Contract as Appendix C or a Certified Check of the Bidder for five (5%) percent of the Bid Amount must accompany the Lump Sum Bid Form. (Appendix A). All questions by prospective bidders concerning this solicitation should be directed, in writing at least three (3) business days prior to the originally scheduled, or, if applicable, postponed bid opening date to E. The bidder is required to execute a pla letter of assent (in the form of appendix l to the construction contract) pursuant to which it agrees to become signatory to, and abide by, the provisions of the pla. This pla letter of assent will be issued at the time of the notice of intent to award letter. The bidder must sign and return this document to the sca along with the required construction performance and labor and material payment bonds. Failure to submit this pla letter of assent will render the bidder to be non-responsive and therefore, ineligible for award of a contract under this solicitation.
