Concrete Maintenance & Repair Services for Public Works Department 1 SITE LOCATIONS & WORK SUMMARY I Boston proper including but not limited to: A 400 Frontage Road, Boston, MA B 280 Highland Street, Roxbury, MA C 170 Hancock Street, Dorchester D 315 Gardner Street, West Roxbury, MA E 58 Gibson Street, Dorchester, MA F 58 Dana Avenue, Hyde Park, MA G 338 East Eagle Street, Boston, MA H 315 Western Ave., Brighton, MA I 430 Canterbury Street, West Roxbury, MA J 500 Meridian Street, East Boston, MA K 165 Alford Street, Charlestown, MA L Beneath North Washington Street Bridge, Boston, MA 2 SCOPE OF WORK I The contractor shall make the necessary work to concrete for the City of Boston buildings and structures, on an as needed basis. II The contractor shall obtain the necessary permits to do the repair work, from any agency having jurisdiction over the project. The contractor shall employ qualified and licensed (or whatever is required by any agency having jurisdiction over the project) repairmen/women to do the work. III The contractor shall provide any and all tools, equipment and material necessary to do the work. IV If the need arises, the contractor may be called to work at any day of the week and to respond to emergencies during the off hours V Contractor's price of each bid unit shall be for the entire work associated with each work unit, per industry standards and applicable rules and regulations in effect. VI The regular hours of operation are Monday thru Friday, 7:00 am thru 3:30 pm; any work outside these hours is considered off hours and will be rated accordingly. VII All material to be used under this contract must be new, un-defective and per code and regulations in effect. VIII Due to the nature of this project, the contractor shall provide the project manager with the name and cell phone number of the contractor's project manager, who will be responsible for the contract. The Contractor's project manager must immediately, or within a reasonable time respond to the City's project manager's call. In response to emergencies, the contractor must be able to mobilize and report to work within the hour any time there is an emergency. The a contractor's failure to respond in a timely manner to the emergency calls may be a good enough reason to disqualify the contractor. Reasonable timely manner is considered within an hour. IV Examples of work that will be completed under this contract are attached as photographs. This contract includes but is not limited to this work. 3 MASTER ITEMS I The quantities below are merely estimates of what may be required and may increase or decrease at the sole discretion of the Official or his representative, as may be necessary. II The items below should be used to calculate the total bid price to be inputted through the Supplier Portal. The number from the 'TOTAL' line should be typed into the supplier portal for a total bid price.
