The bids shall be Unit Price for all work shown in the contract documents and specifications with unit prices for extras as detailed in the Instructions to Bidders in the specifications and submitted in duplicate on the State Board of Accounts Form No. 96. An executed State Board of Accounts Form 96a or similar document shall be required. As a requirement of the Community Crossings Grant funding utilized for this project, the prime contractor to whom the work is awarded must be prequalified with INDOT. Subcontractors can enter into subcontracts up to $300,000 without being prequalified with INDOT; however, the subcontractor is limited to a total of $300,000 in unearned work under contract on any given date with any owner, contractor, or other party.
