Furnishing Refuse and Recycling Collection Services at Various Public Buildings on the Island of Kauai with option for not more than four (4) additional twelve (12) month periods, or part thereof Seven (7) Attachments Offerors must bid on all items to be considered. Offerors shall use provided Offer Form (OF-1 to OF-3) located in header attachments of this solicitation. Bid price per cubic yard shall include all applicable charges, including labor, equipment, transportation, all applicable taxes and any other costs incurred to provide the services specified. Bid price per cubic yard shall be applicable to additional service as requested by the State. Failure to attach required documents shall result in bid disqualification. Award shall be made to lowest responsive and responsible Offeror for bid. Certificate of Vendor Compliance, issued by the Hawaii Compliance Express (HCE) is required for awards of $2,500 or more. If required, it must be submitted within ten (10) working days from the date the bid is closed, otherwise an offer submitted by a responsive and responsible Offeror may not receive the award. Offerors are responsible to make sure Offer Form (pages OF-1 to OF-3), Wage Certificate, and W-9 Request for Taxpayer Identification are submitted as an attachment on HIePRO, otherwise an offer submitted by a responsive and responsible Offeror may not receive the award.
