MAHONING VALLEY SANITARY DISTRICT MAHONING VALLEY SANITARY DISTRICT DAM AND SPILLWAY IMPROVEMENTS ENVIRONMENTAL ASSESSMENT REQUEST FOR PROPOSAL Applicant: Mahoning Valley Sanitary District Point of Contact: Michael D. McNinch, P.E. Chief Engineer 1181 Ohltown McDonald Rd. Mineral Ridge, Ohio 44440 Employer ID Number: 34-6001787 Current Project Title: G-102 Dam and Spillway Improvement Environmental Assessment Project Summary The Mahoning Valley Sanitary District will be performing Dam and Spillway Improvements. This project will provide a complete and comprehensive Environmental Assessment in accordance with FEMA requirements to enable the Mahoning Valley Sanitary District (MVSD) to pursue Hazard Mitigation Grant Program and other funding sources. Project Deliverables * Complete and comprehensive FEMA Environmental Assessment encompassing extent of MVSD Dam and Spillway Improvement Project, including: o Coordinating with FEMA and OEMA to complete document templates o Drafting of all scoping documents and consultations Project Specifics * Significant, permitting, planning, engineering, and on-site surveys are complete. SHPO has concurred on S106. Species surveys and evaluations are complete. Significant information is available on wetland mitigation. Contractor should be prepared to work with FEMA to update information if necessary and present in form acceptable to FEMA. * Contractor must be prepared to do a full evaluation floodplain impact under EO11988 and NFIP. Contractor should be prepared to issue no rise certificate or submit a CLOMR. * Floodplain evaluation will include upstream and downstream effects on FEMA flood hazard areas for a 100 year and Probable Maximum Flood. * Project will require at least 2 public notices in local newspaper with production of affidavit. * Project will require bi-weekly meetings with OEMA, FEMA and MVSD * Contractor must be familiar with 508 compliance for documents so that draft and final version may be posted on line * Contractor must be familiar with NEPA documents for Federal Agencies Contractor must use FEMA provided templates where required Project must be completed by January 1, 2023 to enable use as a component of BRIC application. Proposal must include timeline ensuring completion by that date. Request for Proposal should include information regarding the firm's history, education and experience of key technical personnel; the technical expertise of the firm's current staff; the firm's experience in the similar type of projects; availability of staff; current references; hourly rates; overhead, profit costs and Total Fee proposed. Description of past projects should include the project schedule, status of the project, estimated project costs, actual project cost and the costs associated with any and all change orders on the project. Provide a description of your project approach, not to exceed two pages. Confirm that the firm has visited the site and address your firms technical approach, understanding of the project, innovative ideas, project specific plan for ensuring increased quality, reduced project delivery time and reduced project costs. Significant Evaluation Criteria (30 points) Strength and experience of the assigned staff including subconsultants, technical qualifications of the firm and of the proposed technical staff, as indicated by their past experience with emphasis on current experience. The qualification section of the proposal should reflect demonstrated ability and the offering of professional services covered by the statement of work. (20 points) Firm's current workload and availability of staff. (20 points) Project approach. (30 points) Cost considerations such as the rates charged for each category of employee under the contract, overhead rate, and costs of reimbursable and total fee proposed. Request for Proposal that are incomplete or fail to follow the instructions provided within this notice will not be considered.
