For Solid Waste Collection The bid shall include the following: A. Breakdown of bid by bid item (General, Town Parks, Community Clean-Up, Recycling and Town of Daleville); B. Signed and dated copy of bid specifications signed by authorized agent for Bidder. C. Bid Bond or Certified Check in an amount equal to 5% of the title contract price; D. Certificate of Insurance; E. Financial Statement for Bidder; F. Statement of Experience of Bidder; G. Listing of Equipment that Bidder has available to perform services set forth in contract; H. Completed State Form 96 as prescribed by the Indiana State Board of Accounts, including Non-Collusion Affidavit; The Town of Daleville, Indiana, reserves the right to reject any or all proposals, or to accept any proposal, or to waive all informalities/irregularities inthe bidding. Bids may be held for a period not exceeding sixty (60) days from thedue date for the purpose of revealing and investigating qualifications ofcontractors prior to the award of work.
