Site work for a playground / park / athletic field in Milwaukee, Wisconsin. Completed plans call for site work for a 1,086-square-foot, one-story above grade playground / park / athletic field.

21058 Time for Completion: All work under the base bid must be completed by October 1, 2022. If the Alternate Bids are accepted, the additional scope of work described by the Alternate Bids must be completed by December 1, 2022. Liquidated Damages, per diem: $150.00 The residency requirement for this project is 40% of all hours worked on the project. One quarter of these hours must be attributable to residents living in the following zip codes: 53204, 53205, 53206, 53208, 53210, 53212, 53215, 53233. The SBE requirement for this project is 25% of the contract base bid. The SBE Compliance Plan (Form A) must be submitted with the bid and is an element of bid responsiveness. Incomplete SBE Compliance Plans (Form A) or inaccurate SBE Compliance Plans will not be accepted and will render the bid unresponsive. An active (non-expired) certificate confirming Small Business Enter- prise (SBE) certification issued by the City of Milwaukee Office of Equity and Inclusion for each SBE firm must accompany this form. The SBE firm must be certified at the time of bid opening. For a complete listing of City of Milwaukee certified SBE firms, see the Office of Equity and Inclusion's (OEI) website . If there are any questions regarding SBE firms, please contact the OEI office at 414-286-5553. The apprenticeship requirement for this project is: N/A. If this contract is awarded at $500,000 or higher, 25% of all apprentice labor hours must be worked by apprentices certi- fied for participation in the Residents Preference Program and 40% of all apprentice labor hours must be attributed to apprentices living in the following zip codes: 53204, 53205, 53206, 53208, 53210, 53212, 53215, 53233. The contractor shall specifically note the SBE, residency, and apprenticeship forms for this project. If the forms are not filled out properly, it will be cause for rejection of the bid. All questions should be submitted in writing prior to 12:00 PM on Friday, June 10, 2022 to dpitts@milwaukee.gov. No questions may be answered after that date and time. No questions shall be answered verbally. No verbal information from any source is to be relied upon by any respondent in the development of their Bid. Written questions and responses will be documented by way of addenda, which will be forwarded to all bidders. Bid Security Required: Bond, Certified Check, Cashiers Check or Cash to accompany bid: 10% of the Contractor's Base Bid. For those bids being submitted through Bid Express and using a manual Bond, the contractor may send a signed/executed copy of the bid bond to the Department of Public Works Contract Office by emailing DPWbids@milwaukee.gov. THE EMAILED COPY OF THE BOND MUST BE RECEIVED BY NO LATER THAN 10:30 A.M. ON THE SAME DATE THAT THE BIDS ARE DUE FOR THAT SPECIFIC OFFICIAL NOTICE, or the bid will be consid- ered non-responsive. The original bid bond document must then be submitted by the contractor to the Department of Public Works Contract Office in-person or by mail to 841 N Broadway, Rm. 506, Milwaukee, WI 53202, immediately upon being notified that they are the apparent low bidder. PAYMENT MONITORING REQUIREMENTS: All Contractors awarded a contract valued at $25,000.00 or more are required to participate in training on the City of Milwaukee's B2GNow contract compliance software within 30 days of the contract award date. Throughout the contract term, Contractors are required to regularly provide timely subcontractor payment information in the City's contract compliance software. PAYROLL MONITORING REQUIREMENTS: The Prime Contractor awarded this project is required to participate in training on the City of Milwaukees LCP Tracker Labor Compliance Software after the date of contract award. Throughout the contract term, Contractors are required to regularly provide timely payroll information via LCP Tracker. Please contact the DPW Contracts Office at 414-286-3314 should you have any questions or concerns regarding the training or reporting process. PROJECT DESCRIPTION: A. Victory Over Violence Park involves redevelopment of an existing public park to include new ADA-compliant walkways, a stage, and associated landscape improvements. Work under the contract is generally described as follows: 1. Remove trees and shrubs in accordance with sheet notes and locations shown on vegetation removal plan Sheet CD-100. Debris from removal shall be removed from the site. Holes from stump grinding or removal of roots shall be backfilled with topsoil to be level with adjacent grades. 2. Provide tree / plant protection as noted on Sheets CD-101 and CD-102 and Specification Section 31 1000. 3. Provide and maintain erosion control for earthwork operations to prevent sediment from leaving the construction site. 4. Remove and salvage items such as signs, benches, planters, and utilities as noted on Sheets CD-101 and CD-102. Locate and clean existing storm drain inlet identified on survey at south end of site. Fill former paths removed under prior contract which are no longer intended as walkways. Remove gravel as needed along alley for installation of landscaping. Strip topsoil as necessary for grading operations. 5. Install PVC conduit for future electrical service under paths in locations indicated per Install aggregate paving and concrete pads for site furnishings as noted on Sheets CS-101 and CS-102, including lannon stone walls where indicated. Grade paths per Sheets CG-101 and CG-102. Re-work and/or extend lannon stone wall along street-edge landscape beds as shown on plans, and backfill landscape beds adjacent to stone wall with planting mix. 6. Per Specification for aggregate paths, cross slopes, vertical changes in level and other ADA compliance rules shall be measured, photographed and sent to rrabat@milwaukee.gov prior to job completion. Rebecca Rabatin will inspect slopes for ADA purposes both before and after paving installation so that field orders can be issued while contractor is still on site to ensure ADA compliance. The cost of any/all ADA field orders shall be the responsibility of the Contractor. 7. Soil and surplus earth/gravel material shall be disposed of onsite as shown by the berm expansion along the alley on Sheets CG-101 and CG-102. Priority is to construct the southern berm first, and work towards the north. Cap the berms with 3" of topsoil in lawn areas and 12" of topsoil in landscape beds after disposal operations are complete to meet proposed final landscape grade shown on plans. 8. Notify City representative if materials other than soil and rock are found during excavation operations for direction on disposal. Trash and debris shall not be placed in the proposed fill areas. 9. Provide and install planting soil and landscaping as shown on Sheet L200 and described in Specification Sections 32 9200 and 32 9300. Lawn areas shall be sodded. Blend new landscaping into existing adjacent beds to leave site in a clean condition. 10. Alternate Bid Items are as follows and as shown on the title sheet of the Drawings: a. Alternate Bid #1: Site Furnishings i. Base Bid: Install concrete pads only for future installation of surface mounted furnishings by Owner. ii. Add Alternate Bid #1.A: Provide and install three (3) game tables with seats. iii. Add Alternate Bid #1.B: Provide and install four (4) benches. iv. Add Alternate Bid #1.C: Provide and Install three (3) bike racks. b. Alternate Bid #2: Stage i. Base Bid: Perform earthwork only to reconfigure berms in area shown on Drawings. Sod entire area of disturbance, except for trees as installed under the base bid. ii. Add Alternate Bid #2: Construct stage as shown on attached architectural plan set. Install aggregate paths and concrete bench pad in area outlined on plans. Install topsoil and mulch in landscape bed north and west of stage. (Plants included in Alternate Bid #3, below). c. Alternate Bid #3: Landscape i. Base Bid: Provide and install topsoil and mulch in all landscape bed areas and five (5) deciduous trees as shown on Sheet L200 - Site Planting Plan. ii. Add Alternate Bid #3: Provide and install all additional plant material as shown on Sheet L200 - Site Planting Plan, including evergreen shrubs, deciduous shrubs, perennials, ornamental grasses, and vines.
