Site work, paving and renovation of a mixed-use development in Salem, Virginia. Completed plans call for site work for a sidewalk / parking lot; for paving for a sidewalk / parking lot; water / sewer project; and for the renovation of a medical facility.
Project Description: IAW FAR 36.204 (b), Magnitude of construction is between $25,000 and $100,000 This project is to excavate on both side of the tunnel to expose it and the expansion joint providing the interface to bldg. 143. Once excavated, the tunnel is to be repaired and the expansion joint replaced. The tunnel will then be dampproofed and the excavation filled back in. 2. Primary Bid Items: A. In preparation for excavation, the Contractor shall determine the extent of their intended excavation and perform an on-site utility survey to locate any utility and communication lines within the intended excavation perimeter as well as an additional 10 feet in all directions. NOTE: Any plans or diagrams provided by the VA shall be considered unreliable, and contractor is responsible to field verify all conditions. B. Unless buried utilities or communication lines prevent excavation as planned, the Contractor shall then excavate the tunnel the full height of the tunnel a minimum of 10 feet from the interface with bldg. 143 towards bldg. 4. on both sides to fully expose any damage associated with the leak. NOTE: The tunnel shall be supported as necessary to avoid damage for the duration of this project. Once the excavation if complete the contractor shall replace the expansion joint between bldgs. 143 and the tunnel and assess and repair any damage to the tunnel. The construction of the tunnel is detailed on the plans included in this solicitation. NOTE: For project proposal estimation, the contractor shall assume the tunnel requires the following items will need to be repaired: a. 200 LF of crack repair b. 100 sqft of concrete spall repair and patching c. 300 SF of water/damp proofing d. 150 LF expansion joint replacement Once the tunnel repairs are complete, the Contractor shall dampproof the tunnel to its full height on both sides and then verify and demonstrate the functionality of the damp proofing to the VA's satisfaction. Once the VA has approved the damp proofing test the Contractor shall fill the excavation and remediate any disturbed soil and pavement disturbed as part of this project. NOTE: All work shall be performed to minimize disruption to VA Medical Center activities, operations, and movement. 3. Project Specifications and Other Requirements: These documents and all national, state, and local codes adopted and used by the VA shall be incorporated into the Corrective Action Plan for this project. At a minimum, the following references shall be utilized for planning and design purposes. a) VA Supplied Construction Specifications b) Life Safety Codes c) VADEQ SWPPP Guidelines 4. Project Schedule: This project has been estimated to be completed in 175 days after contract kickoff. SUBMISSION OF BIDS: The bidder shall submit: o A completed and signed Standard Form 1442. o A completed copy of the Representations and Certifications (See FAR 52.204-8 Annual 3 Representations and Certifications (MAR 2012)). o Documentation of their safety record. o All forms and/or certifications as directed in the solicitation, including the specifications. o A copy of VetBiz registration o A copy of System Award Management (SAM) registration to include vendors Unique Entity Identifier (UEI) o One (1) electronic/digital (pdf) copy of the Standard Form 1442 signed; copy shall include signed Standard Form 1442, Certificate copy of SAM registration, Certification copy VETBiz registration, copy of Standard Form 25 signed, copy of bid breakdown by divisions, copy of past performance that refers to excavation projects, signed copy of the VAAR 852.219-77 VA Notice of Limitations on Subcontracting. SAFETY AND ENVIRONMENTAL RECORD: In order to be eligible for consideration and award, the contractor shall have no more than three serious, or one repeat, or one willful OSHA or EPA violation(s) in the past 3 years and have an Experience Modification Rate (EMR) of equal to or less than 1.0. *The value for this project is based on a financial range.
