The City of Sierra Vista is requesting quotes from qualified, licensed contractors (when applicable) who can provide the goods and services specified in this Request for Quotation. A Contractor desiring to respond to this Request for Quotations shall include a fully executed Non-Collusion Affidavit, and acknowledge all addenda (if applicable). The City of Sierra Vista has been awarded funding under The Emergency Home Repair and Disabilities Modification Program ("Emergency Program" or "Program") that assists homeowners in Sierra Vista to perform emergency repairs on substandard housing conditions. Substandard conditions include those posing a serious threat to the health, safety or welfare of the household or affecting the immediate livability of the home. The residence located at 125 E. Keating Street, Sierra Vista AZ 85635 has been determined eligible to receive such repairs under the program. The maximum amount of assistance per household, may not exceed $10,000. The project must commence within 14 days of Notice To Proceed and the project must be completed within 30 days after project start.
Supply chains issues: If the proposed timeline in the scope of work is not reasonable due to market conditions with
equipment or other variables, please submit an alternate timeline with the quote with an
explanation. Submit you alternative timeline under "Additional Documentation" in the Vendor
