Desert Regional Center, (State of Nevada), is seeking proposals from qualified contractors to establish a contract with a credible and efficient service provider for glass repair or replacement services on an as needed basis at various locations within Desert Regional Center. It is Desert Regional Center's intent to enter into a contract with a vendor, acting as an independent contractor, who has experience in meeting the needs of the agency. The needs will vary but may include at a minimum: an on-site inspection to determine the type of glass/windows to be repaired or replaced, preparation of written estimates, coordination of installation and debris removal. It is anticipated the contract, administered by Desert Regional Center, will commence in November 2022, upon the Board of Examiner's approval. The term will be for an initial two (2) year period. The contract may be extended for an additional two years if performance is satisfactory. Desert Regional Center does not guarantee any minimum volume of work. The Contractor shall provide all necessary equipment and materials. Compensation shall be at negotiated contracted rate(s) for labor and materials. Services shall be provided on an as needed basis as determined by Desert Regional Center. The Contractor shall be paid only for the services performed. Your proposal should include at minimum: o A copy of an applicable city and/ county business license/certification; o A copy of your State Business License; o A copy of your Certificate of Insurance; o Hourly rate for labor - including after hours and emergency; o Materials mark up; o Company ownership and length of time in business; o Contact person and telephone number, email address and mailing address. Attached to this request is the State's standard Contract for Services of Independent Contractor form for your review. Please pay special attention to Section 12 and Attachment BB, as this identifies the insurance limits contracted vendors are expected to maintain for the life of the contract. Evidence of the required insurance in the form of Insurance Certificate(s) and Endorsements are required to be submitted to the contracting agency prior to the start of the work of the contract.
