To provide general equipment maintenance and repair. Award will be based on the lowest price. 1. PRICING MUST BE VALID from July 2022 - June 2023. 2. AWARD WILL BE BASED ON LOWEST BID 3. Bidders must respond to all items on the bid to be considered for contract award. 4. Vendor shall maintain sufficient stock of parts to perform designated service to complete services within two (2) business days of receipt of verbal or electronic order. If service and parts are not available at successful vendor's facility, the City reserves the right to obtain those parts and service from other sources. 5. All items listed in the Line Items represent typical equipment repair services needed and will be used for bid comparison purposes only. Street Maintenance Division may, in accordance with the specifications, request additional materials, parts, or supplies at the Division's sole discretion and payment will be made for the amount of materials actually provided as determined by the Division and accepted at the unit prices and labor noted in the quote. Actual purchases may vary. 6. For other items not listed under Line Items, bidders are asked to submit a list of services and labor offered by the bidder in a PDF file. File must contain the list of services provided and corresponding price, and must be submitted with quote. If government discounts apply, please include with bid submittal. 7. Awarded vendor will be required to provide the following: (a) W-9 Form, (b) completed vendor questionnaire form for new vendors, (c) insurance documents, if vendor is coming on site, please refer to the attachments for samples of the forms, (d) upload invoices electronically to the City's website at https://invoice.cityofpasadena.net/, and (e) submit invoices no later than 7 days after the required delivery or completion date. Invoice must clearly specify the PO number, detailed description of goods and/or services as applicable (i.e. unit price, quantities billed, extended totals, service date, service location, hourly rate, hours billed), and any applicable taxes.
