Quotes must meet criteria in the Statement of Work (SOW), dated 9 June 2022, and associated diagrams within Attachment 1. Four (4) manufacturers (Shaw Inc, Engineered Floors LLC/J&J Flooring Group, Mohawk Carpet Distribution Inc, and CMA Inc) meet the Air Force's established technical carpet requirements and adhesives. The vendors have established terms and conditions to potential installers/dealers. Offerors must use these vendors for the carpeting and adhesives. Contact the manufacturers for material quotes. Offerors must list the price for the carpet and adhesives from each manufacturer contacted, and which manufacturer the offeror intends to use. Provide the price for the carpet and adhesives in your quote. The contact information for each of the four manufacturers, the respective Air Force Catalogs, and an example pricing format to include in offeror quotes is provided in Attachment 2. The Contractor shall furnish all labor, equipment, materials, tools, and transportation necessary to accomplish the work. 3. All debris shall be disposed off base. 4. All of the existing furniture in the work areas will be removed by others prior to the start of work. 5. The work will be completed in four (4) phases. There will be a delay of about one week between phases. 6. Two floor plans are attached. Floor Plan 2 shows the square foot (SF) area of each room. Floor Plan 3 has green and blue squares showing where the new carpet and VCT tile will go. 7. The rooms will be identified according to their square foot sizes listed on Floor Plan 2. 8. Demolition Work: Remove and dispose the carpet in the following rooms: 450, 756, 212, 188 (hallway), 141, 122, 257, 193, and 183. Remove and dispose the ceramic tile in the following rooms: 188 (vestibule), 34, 42, 125, 120, 142, 103, 618, 77, 179, 139, 219, 269, 226, 159, 192, and 34. The following rooms have painted concrete floors: 755, 122, 119, 155, and 262. Remove and dispose the wall base in all the rooms. 9. Clean and prepare the concrete floor as required after demolition operations are complete. 10. New Flooring Work: Install new modular carpet tile in the following rooms with green squares: 450, 756, 141, 122, 193, and 257. Install VCT tile in the remaining rooms with blue squares. 11. In Room 1068, there is an office that contains carpet. This is not shown on Floor Plan 2, but is shown on Floor Plan 3 as Room 133 (Debrief Room). Remove and replace the carpet in this office. This office space is 20' x 20' in size. 12. Do not perform any work in Rooms 131 (Server) and 132 (Mis Lab) shown on Floor Plan 3. These rooms are not shown on Floor Plan 2. 2 13. There is a janitor's closet next to Room 226 (women's bathroom). This is labeled as Room 124 on Floor Plan 3. Remove the ceramic tile and install VCT tile in this closet. 14. For the carpet tile, the contractor must use one of the following four carpets. All carpet shall have a cushion backing built into it. A. J+J Flooring Group: Impulse III 2433. B. Shaw Inc: Gradient Ecoworx 59534. C. Mohawk Group: BT443 First One Up II Tile. D. CMA Milliken: Nordic Stories Tectonic. 15. The VCT tile shall be 12" x 12" in size, and 1/8" thick. 16. Install new 4" high vinyl wall base in all the rooms that will have new flooring. 17. Install vinyl transition strips across all joints between the new carpet and tile. 18. Deliver 20 replacement carpet tiles, and 20 replacement VCT tiles to Bldg 894. 19. There is no asbestos in the work area. 20. All installation work shall be in accordance with the manufacturer's instructions. 21. Seal off the work areas with plastic to prevent dust and debris from entering other areas of the building. Install barricades as required. Seal off HVAC vents as required. 22. The Contractor will be allowed to park storage trailers and equipment on site at approved locations. 23. Provide material submittals for approval. The colors for the carpet, VCT, transition strips, and wall base will be selected at that time. Do not order any materials until the submittals have been approved. 24. Perform a complete clean-up of the work area after each phase is complete. 25. Repair any damage that may occur during the course of the work. .
