The scope of work includes performing a mechanical integrity test (MIT) at the City of Hollywoods Class I Injection Well N. 1 IW-1 located at 3441 Hollywood Boulevard. This facility is operated by the CITY under a Class I Injection Well permit (Florida Department of Environmental Protection [FDEP] Underground Injection Control [UIC] Permit No.: 304444-002-UO/1X). An MIT of this facility is due by December 20, 2022 as required by Chapter 62-528, Florida Administrative Code. Injection wells are required to demonstrate mechanical integrity via testing a minimum of once every 5 years. The MIT consists of internal testing to demonstrate the absence of leaks in the injection well casing, and external testing to demonstrate the absence of fluid movement behind the injection casing. The MIT will be performed in accordance with technical Specifications 11 56 10 and the MIT plan for IW-1, as approved by FDEP Q & A deadline: Jul 13, 2022 5:00:00 PM EDT
