Scope: Cut grass & remove all trash & debris from lot A certificate of insurance evidencing the following: General Liability coverage in the amount of not less than $1,000,000.00 per occurrence including products and completed operations. Coweta County, Georgia and the Coweta County Board of Commissioners are to be added as additional insured to the General Liability policy for premises/operations and products/completed operations. An aggregate of $1,000,000.00 should be stated on the Certificate of Insurance. Workers' compensation coverage with employers' liability limits set at statutory minimums. Automobile liability including non-owned automobile liability of not less than $1,000,000.00. Completed Contractor Affidavit (Affidavit under O.C.G.A. 13-10-91(b)(1)) unless the contractor/vendor meets one of the following conditions The contractor/vendor has no employees (in which case they must present an approved state issued identification card/drivers' license from an approved state as provided on the Attorney General's website) The contract is with an individual licensed under Title 26, Title 43, or the State Bar of Georgia who is in good standing and that individual is performing that service.
