Site work and new construction of a medical facility in Paw Paw, West Virginia. Working plans call for the construction of a one-story above grade medical facility; and for site work for a medical facility.

The Project includes the following Work: This project is for the construction of a new one story, concrete slab on grade, wood framed medical clinic on the designated area of the existing MCHC site located in Paw Paw, WV. In general, the exterior finishes will be conventional shingle roofing, vinyl siding with aluminum soffits, trim, gutters and downspouts and masonry base walls, insulating Fibrex windows, and painted hollow metal or thermally broken aluminum entrances. Interiors are basically painted 5/8" gypsum board partitions with sound batt insulation, painted hollow metal door frames, stained solid core wood doors, VCT or seamless vinyl flooring and base, 2x2 suspended acoustical lay-in ceilings, and casework, equipment and fixtures as indicated. Heating and cooling will be with a standard forced air system, and plumbing will be standard commercial grade fixtures. Electrical and lighting are also typical commercial grade installations with LED lighting. The main specialty area required for this project is the Xray Room including lead lined gypsum board, doors and hardware and screen walls. Site work will consist of concrete entrance sidewalks and curbs from asphalt paving parking that expands off of existing parking areas and seeding for finished lawn area. All site utility extensions to public utilities are also to be included in the contract. Project has been cancelled. Bids are requested for the following Contract: MCHC Medical Building The Work will be substantially completed within 210 calendar days after the date when the Contract Times commence to run and completed and ready for final payment within 30 days after the date when the Contract Times commence to run. Liquidated damages shall be $50.00 per day. Printed copies of the Bidding Documents may be obtained from the Issuing Office by paying the amount of each set listed below. Bidding Documents may be purchased from the Issuing Office during the hours indicated above. Cost does not include shipping charges. Upon Issuing Office's receipt of payment, printed Bidding Documents or electronic documents will be transmitted to the prospective Bidder. A two-envelope system will be used. Envelope No. 1 must have the following information presented on the front: Name and address of Bidder Bid on Contract - MCHC Medical Building Received by the Mountaineer Community Health Center Envelope No. 2 labeled "Bid Proposal" shall also be placed inside of Envelope #1. Envelope No. 1 will be opened first, and the Bid Opening Requirement items checked for compliance as outlined on the Bid Opening Checklist on page BOR - 1 of these contract documents. If such documents are found to be in order, Envelope No. 2 "Bid Proposal", will then be opened and will be publicly read aloud. If the documents required to be contained in Envelope No. 1 are not in order, Envelope No. 2 "Bid Proposal" will not be opened, and the Bid will be considered non-responsive and will be returned to the Bidder. A bidder may not withdraw his bid for a period of ninety (90) days after the date set for the opening of bids. Bids shall be accompanied by a certified check or bid bond payable to Mountaineer Community Health Center in an amount equal to five percent (5%) of the base bid. Contractor is required to provide the Drug Free Workplace Certification in accordance with WV Code 21- 1D-5. All Bidders are required to have a West Virginia Contractor's License per Section 21-11-11 of the West Virginia Code. Bids received after the scheduled closing time for the reception of bids will be returned unopened to the bidders. Mountaineer Community Health Center reserves the right to reject any and all bids.
