Due to the COVID-19 protective measures, all visitors must wear a mask while in the facility viewing the site. While outside, on the grounds, keep in mind 6' for social distancing is a safe option. Location: Hunter Holmes McGuire VA Medical Center, Fisher House 1, Bldg. 510 1201 Broad Rock Blvd, Richmond, VA 23249 Questions are due no later than Friday, July 22, 2022 @ 3:00 PM EST Quotations, breakdowns, and all required documents are due no later Monday, August 8 GENERAL INTENTIONS Contractor will provide services to include all tools, labor, materials, equipment, and supervision necessary to Fisher House 1 Interior Painting and Repairs, Building 510, Richmond VA Medical Center campus, Richmond, Virginia. SITE VISIT Offerors are encouraged to inspect the site where services are to be performed and to satisfy themselves regarding all general and local conditions that may affect the cost of performing these services. In no event shall failure to inspect the site constitute grounds for a claim after contract award. There will be ONE formal site visit on Friday, July 15, 2022, at 2:00 PM (EST) REQUESTS FOR INFORMATION (RFI): To obtain clarifications and/or additional information concerning the contract requirements or specifications, submit a written RFI via email to Denise.Morman@va.gov. RFIs will be responded to as necessary in amendment format which will be posted under Contract Opportunities on www.SAM.gov. The deadline for submission of RFIs for this solicitation is Friday, July 22, 2022, at 3:00 PM (EST). No questions will be answered after this date/time unless determined to be in the best interest of the Government as determined by the Contracting Officer. Telephone RFIs will not be accepted or returned. BASIS FOR AWARD Contract award will be made to the responsive, responsible offeror submitting the best value offer in accordance with FAR Part 13 - Simplified Acquisition Procedures. All offers shall include line-item pricing, as well as itemized breakdown of material, equipment, labor, overhead and profit, to facilitate price and technical evaluation. Offers shall include a brief synopsis of how the offeror will proceed with and complete the work. Offers shall include a completed SF 1442 solicitation, including fill-in provisions and acknowledgement of amendments. The Government will evaluate offers to identify the most advantageous technical response that is fair and reasonable in price when compared to the Independent Government estimate and other offers received. RESPONSIBILITY REQUIREMENTS A Determination of Responsibility shall be accomplished for the apparent awardee prior to processing the award. The above information, along with other information obtained from Government systems, such as the OSHA and EPA online inspection history databases will be used to make the Determination of Responsibility. Failure to affirm being within the guidelines above or submit this information may result in a determination of "Non-Responsibility" for the offeror. NOTE: Any information received by the Government that would cause for a negative Determination of Responsibility may make the offeror ineligible for award. This requirement is applicable to all subcontracting tiers, and prospective prime contractors are responsible for determining the responsibility of their prospective subcontractors. Quotes Due on or before Monday, August 8, 2022, at 4:00 PM (EST) EVALUATION--SIMPLIFIED ACQUISITION PROCEDURES FAR PART 13 A.4 EVALUATION PROCEDURE Basis for Award. The Government will issue a contract to the responsible quoter whose quotation conforming to the solicitation will be most advantageous to the Government, based on price alone. Evaluation Approach. The Government will evaluate quotations using the comparative evaluation process outlined in FAR 13.106-2 where quotations will be compared to one another to determine which provides the lowest price to the Government. Each response must meet the minimum requirements of the solicitation. The Government is not requesting or accepting alternate quotations. The evaluation will consider price alone. The Government will evaluate the price by adding the total of all line items priced and provided on the Quote. A.5 INSTRUCTIONS TO OFFERORS/SUBMITTAL REQUIREMENTS 1. Solicitation Number and Project Title 2. Company information to include: UEI number, DUNS Number, Cage Code, Tax ID Number, General Contractor's License number and State of Licensure, principal points of contact with addresses, phone numbers and email addresses. 3. Complete the SF 1442, Blocks 14, 15, 17, 19, 20A, 20B, and 20C to submit the Quote. All sections of the RFQ, excluding Instructions to Offerors and Evaluation Procedures Sections, constitute the model contract. 4. Quote with breakdown of labor, material, equipment, overhead and profit. An example of the required information has been provided. 5. The Offeror shall submit the following information: (For a partnership or joint venture, the following submittal requirements are required for each Contractor who is part of the partnership or joint venture and for the entity itself. EMR shall not be submitted for subcontractors.) a. OSHA and EPA Violation(s): Offerors shall provide Summary of Work-Related Injuries and Illnesses, affirming that the Offeror has no more than three serious, or one repeat, or one willful OSHA and EPA violation(s) in the past 3 years b. Experience Modification Rate (EMR): Offerors shall provide documentation to show that the offeror has an Experience Modification Rate (EMR) of equal to or less than 1.0. 6. Acknowledgement of Amendments A.6 STATEMENT OF WORK STATEMENT OF WORK Building 510 Fisher House 1 Painting and Repairs 1. General Requirement--The Contractor will provide all transportation, materials, parts, tools, specialized equipment, labor, supervision, and technical expertise to paint interior of facility and perform repairs listed in Section 5.1 at 1201 Broad Rock Blvd Richmond, VA 23249 2. Coordination. The engineering Carpentry shop will serve as the focal point and point-of-contact (POCs) for this contract. The Contracting Officer Representative for this service will be George Martin and he can be reached at George.Martin@va.gov or 804-675-5201. The alternate POC will be Allen Miller who can be reached at Allen.Miller@va.gov or 804-675-7506. 2.1. The contractor must pre-coordinate and preschedule work with the POCs. This work can only be performed during normal business hours, M thru F 800AM to 430 PM. 3. Government Provided Tools, Materials and Equipment. The Federal Government will not provide any tools, parts, materials, and equipment. This is a turn-key project with the vendor providing all necessary tools, parts, materials, and equipment. 4. Specifications. The following specifications will apply: Report to the Hunter Holmes McGuire VA Medical Center, Engineering Carpentry Shop POCs/CORs before starting work each day. 5. Scope of Work: All work will be accomplished in-accordance with applicable Life Safety and OSHA codes and regulations, Available in Additionally, the Department of Veteran Affairs guidelines and policies will be adhered to. Duties to include: 5.1. Painting the Interior Ceiling of Facility (27,000sqf) o Prepare all surfaces for painting to include spackling, filling holes, sanding, blue taping, etc. o Apply Primer coat of paint o Apply Finishing coat of paint 5.2. Repair Cracks in the Ceiling (1000sqf) o Inspect all cracks and determine repair requirements o Prepare all surfaces for repair, include spackling, filling holes, replacing drywall, o Apply Primer coat of paint as required o Apply Finishing coat of paint o Dry Wall repair Capped at 1000 square feet o If additional repairs are found and before any work is done, please notify COR and CS. 5.3. Repair Door Jams in the Hotel Rooms o Detach and Reset 2 door jams and 4 Doors o CO required to replace door frames and door hardware as required o Capped at 2 door Jams (CO required for additional Jams) o If additional replacements of Jams and Doors are found and before any work is done, please notify COR and CS. o If additional cost to prep and paint new jams and doors are found and before any work is done, please notify COR and CS. 5.4. Repair the Crown Molding (8 Joints) o Repair crown molding as required (8 joints) > Apply Primer coat of paint as required o Apply Finishing coat of paint 5.5. All work-related debris will be carried off station by the contractor. 5.6. Period of Performance: 60 days. *The value for this project is based on a financial range.
