Kilbourn Ave. to W. Richardson Pl. N. 37th St. - W. Clarke St. to W.Center St. E. Clarke St. - N. 1st St. to N.Palmer St. W. Clarke St. - N. 37th St. to N.38th St. W. Hadley St. - N. 25th St. to N.27th St. W. Locust St. - N. 17th St. to 15'+/- E. of N. 20th St. Time for Completion: All work inE. Clarke St. and N. 1st St. shallbe completed by October 28, 2022.All other work shall be completedby June 16, 2023. All work shall becontinuous and the number ofallow able work days shall be 70days. Liquidated Damages, per diem:$1000 The residency requirement for this project is 40% of all hours worked on the project. One quarter of these hours must be attributable to residents living in the following zip codes: 53204, 53205, 53206, 53208,53210, 53212, 53215, and 53233. The SBE requirement for this project is 15% of the contract base bid. The SBE Compliance Plan(Form A) must be submitted with the bid and is an element of bid responsiveness. SBE certificates must be submitted with the Form A. Incomplete SBE Compliance Plans (Form A) or inaccurate SBE Compliance Plans will not be accepted and will render the bid unresponsive. Vienna Ave. N. 25th St. - 125' +/- N. of W.Capitol Dr. to W. Hope Ave. S. 90th St. - W. Adler St. to 340'+/- S. of W. Adler St. W. Blue Mound Rd. - N. 63rd St.to N. 67th St. W. Vienna Ave. - 245' +/- E. of N.24th Pl. to N. 24th Pl. W. Wisconsin Ave. (S/S) - N.55th St. to N. Hawley Rd. Time for Completion: All work in West Blue Mound Road shall be completed by June 2, 2023. All other work shall be completed by November 23, 2022. All work shall be continuous and the number of allow able work days shall be 75 days. THE EMAILED COPY OF THE BOND MUST BE RECEIVED BY NO LATER THAN 10:30 A.M. ON THE SAME DATE THAT THE BIDS ARE DUE FOR THAT SPECIFIC OFFICIAL NOTICE, or the bid will be considered non-responsive. The original bid bond document must then be submitted by the contractor to the Department of Public Works Contract Office in-person or by mail to 841 N Broadway, Rm. 506, Milwaukee, WI 53202, immediately upon being notified that they are the apparent low bidder. PAYMENT MONITORING REQUIREMENTS: All Contractors awarded a contract valued at $25,000.00 or more are required to participate in training on the City of Milwaukee's B2GNow contract compliance software within 30 days of the contract award date. Throughout the contract term, Contractors are required to regularly provide timely subcontractor payment information in the City's contract compliance software. PAYROLL MONITORING REQUIRE MENTS: The Prime Contractor awarded this project is required to participate in training on the City of Milwaukee's LCP Tracker Labor Compliance Software after the date of contract award. Throughout the contract term, Contractors are required to regularly provide timely payroll information via LCP Tracker.
