PROJECT TITLE AND NUMBER: Correct SPS Deficiencies - Marion, 610A4-20-513 PROJECT LOCATION: VA Northern Indiana Health Care System (VANIHCS), 2121 Lake Ave,Fort Wayne, IN 46805 MAGNITUDE OF CONSTRUCTION: Between $100,000 and $250,000 TYPE CONSTRUCTION: Commercial and Institutional Building Construction NORTH AMERICAN INDUSTRY CLASSIFICATION SYSTEM CODE: (NAICS): 236220 SMALL BUSINESS SIZE STANDARD: $39.5 Million PERIOD OF PERFORMANCE: 95 calendar days from receipt of Notice to Proceed. WAGE DETERMINATION: Department of Labor General Decision Number IN20220002 is included as an attachment to solicitation and is applicable to any resultant contract. If necessary, subsequent revisions will be posted to the solicitation via amendment prior to bid opening. Revision may also be applicable prior to any contract award. Wage decisions are available at https://sam.gov/. REQUESTS FOR INFORMATION (RFI): To obtain clarifications and/or additional information concerning the contract requirements, specifications, and/or drawings, submit a written RFI utilizing the RFI Form attached to this Solicitation and send it via email to eric sweatt@va.gov. The subject line of the email must read 'RFI - 36C25022B0062 Project 610A4- 20-513 Correct SPS Deficiencies - Marion. RFIs must be specific identifying section, paragraph, and page no. of the specifications or cite the drawing number and must be in question format. All RFIs submitted shall include the solicitation number and title, contractor name, city, state, telephone, email address, date submitted, and the RFI question(s). RFI 36C25022B0062 Page 9 of 56 responses will be posted to https://sam.gov/ as necessary in amendment format. The deadline for submission of RFIs for this solicitation is the close of business on August 2, 2022. No questions will be answered after this date unless determined to be in the best interest of the Government as deemed by the Contracting Officer. Telephone requests for information will not be accepted or returned. TYPE OF SET-ASIDE THIS SOLICITATION IS A 100% SET-ASIDE FOR SERVICE-DISABLED VETERAN-OWNED SMALL BUSINESSES (SDVOSB) SEE THE SOLICITATION FOR ELIGIBILITY REQUIREMENTS. DATABASE REGISTRATION INFORMATION SYSTEM FOR AWARD MANAGEMENT REGISTRATION: The System for Award Management (SAM) is the Official U.S. Government system that consolidated the capabilities of the Central Contractor Registration (CCR including FedReg), Online Representations and Certifications Application (ORCA), and the Excluded Parties List System (EPLS). Federal Acquisition Registrations require that federal contractors register in SAM at www.sam.gov and enter all mandatory information into the system. THE EXCLUDED PARTIES LIST SYSTEM (EPLS): To ensure that the individuals or businesses providing services under the contract have not engaged in fraud or abuse regarding Sections 1128 and 1128A of the Social Security Act regarding federal health care programs, the contractor is required to check the Excluded Parties List System (EPLS) located at www.sam.gov for each person or business providing services under this contract. During the performance of this contract, the Contractor is prohibited from using any individual or business listed on the List of Excluded Individuals/Entities. VETS-4212 REPORTING REQUIREMENT: Contractors are required to submit a required annual Form VETS-4212, Federal Contractor Veterans' Employment Report in all cases where the contractor or subcontractor has received an award of $150,000 or more in any fiscal year. Contracting Officers are prohibited from awarding a contract to a contractor that has not submitted a required VETS-4212 Report with respect to the preceding fiscal year if the contractor was subject to the reporting requirement of 38 U.S.C. 4212(d). Submit this report as soon as possible, if not already submitted, to avoid delays in the contract award process. Correct SPS Deficiencies Project: 610A4-20-513 VA Northern Indiana Health Care System (VANIHCS) Ft. Wayne, IN Project Background: The new Sterile Processing Service (SPS) area has struggled with maintaining the proper negative and positive pressure in rooms since it was built. This project will correct the HVAC deficiencies inside of SPS. Statement of Work: Contractor will provide all labor, equipment, materials, and supervision necessary to Correct SPS Deficiencies, Building 1 at the Fort Wayne campus of the VANIHCS following the A/E s design drawings to include but not limited to the following: Correct negative and positive pressures inside of SPS, add a door and several door closures to the SPS breakroom, add a door and door closure to the scope decontamination room, build an equipment storage room inside of SPS and correct the balance in the warehouse and corridor C3 and conference room B106E, and SPS staff office. The SPS and warehouse areas were constructed post-2015. The VA shall provide the most recent hazardous survey. This report is merely a starting point and should not be considered to have captured every area. The project is divided into 6 areas per the A/E s drawings. The areas do not have to be completed in sequence and can be completed concurrently. As a working hospital, the adjacent spaces and functions will need to be maintained and a high degree of coordination, scheduling, and phasing will be required to allow the continuation of the existing hospital services during the entire construction project. Corridors shall not be blocked during normal business hours. Phase I shall be bid as after-hour or weekend work. Working inside of the SPS department will require SPS to be shut down. Work is expected to take 45 calendar days inside of SPS and another 45 calendar days of work outside of SPS. The total project should not exceed three months of construction. All work is to be completed at the VA Northern Indiana Health Care Fort Wayne Campus, 2121 Lake Ave, Fort Wayne, IN 46805. Work to be completed within 95 calendar days from notice to proceed. Justification of Work: This project is designed to correct the SPS room pressure deficiencies to allow the VA and SPS to work in a more climate-controlled cleaning environment. This in turn will provide a world-class scope cleaning environment to ensure the cleanest possible items are being used on our Veterans. *The value for this project is based on a financial range. The value is listed as the highest possible cost from the range provided by a stakeholder or official project document.
