Site work and paving for a sidewalk / parking lot in Hollister, California. Completed plans call for site work for a sidewalk / parking lot; and for paving for a sidewalk / parking lot.

Sealed proposals will be received by the Board of Directors of the San Benito Health Care District ("SBHCD") located at 911 Sunset Drive, Hollister, California, for the furnishing of all labor, materials, equipment and services to SBHCD necessary for and incidental to the construction. This project consists of the general construction of: The Project Scope of Work includes: 6,000 sqft of asphalt repair, apply asphalt sealer to approximately 180,0000 sqft and restripe parking lines, curbs and ADA spaces as required. Contractors shall walk and conduct site survey validation during the mandatory job walk. Bids. Bids will be referred to the next SBHCD Board of Directors meeting for appropriate action. Each bid shall be in accordance with the Documents prepared by Adrian Flores, the Director of Plant Operations and shall be submitted on forms contained in the Specifications. Note: Bids submitted orally or by telephone, telegraph or facsimile will be considered nonresponsive and invalid and will not be accepted. Questions. All requests for interpretation of the site documents, and/or specifications shall be submitted to the Director of Plant Operations, Adrian Flores at aflores2@hazelhawkins.com in writing via email. No telephone requests shall be accepted. All requests shall be submitted to the Director of Plant Operations no later than five (5) days prior to the bid opening date. Requests for information can be sent to, aflores2@hazelhawkins.com. Each bid must be accompanied by a Cashier's Check, Certified Check, or Bid Bond, made payable to San Benito Health Care District, in the sum equal to of the amount of the Base Bid. This check or bid bond shall guarantee that the Contractor will sign a contract and furnish bonds as specified within ten (10) days after receipt of notification of bid acceptance. If the bidder defaults in executing and delivering the Agreement and Bond within the time specified, the check or bid bond will become the property of SBHCD. Payment and Performance Bonds. Each bond must meet the statutory requirements for a public construction project as set forth in California Civil Code Sections 9950 and 3248. The bonds shall be secured through a surety company approved by SBHCD and paid for by the General Contractor. Bid Acceptance/Rejection. SBHCD reserves the right to reject any or all bids and to waive any informalities in the bidding, or in any bid received. For the purpose of determining the lowest bid price, the lowest bid shall be the lowest total of the base bid plus those additive or deductive items submitted as alternate bids taken in order from the written list of requested alternate bids provided by the Architect, if SBHCD chooses to include or exclude such alternate bids. No bid shall be withdrawn for a period of sixty (60) calendar days subsequent to the opening of bids without the consent of SBHCD. Bidder Requirements. Each bidder must be licensed to do business in the State of California. Every bidder must possess a valid contractor's license at the time of submission of his/her bid. This license must be valid at the time the contract is awarded through the completion of the project. Each bidder shall include with his or her bid a list of the name and location of the place of business of each subcontractor who will perform work, labor or render services to the prime Contractor in the construction of the project and the portion of work to be performed by each subcontractor. This list must comply with the Subletting and Subcontracting Fair Practices Act at Public Contracts Code Section 4100 et seq. Special Qualifications. Prevailing Wage. Minimum prevailing wage rates are required to be paid for each craft, classification, or type of worker needed to execute the Contract. Copies of such minimum rates are on file at the Administration office of SBHCD, and are available to any interested party upon request. Mandatory Job Walk. A bidder representative is required to attend this job walk and should meet at the Plant Operations Main Entrance at the Support Services Building, 911-C Sunset Drive, Hollister, CA 95023 at the designated date and time.
