Site work and paving for a mixed-use development in Bellevue, Washington. Completed plans call for site work for a sidewalk / parking lot; for paving for a sidewalk / parking lot; and water / sewer project.

12-24N-5E GRID F-14 The Citys Compliance Officer is the Purchasing Manager, and specific questions pertaining to this section may be directed to the Purchasing Division at (425) 452-7246. Work contemplated to be performed under this contract is as follows: o Gravity sewer pipe repair and replacement (see Measurement and Payment Schedule A) and gravity storm pipe repair and replacement (see Measurement and Payment Schedule B) activities requiring excavation at 38 total site locations, a total of 42 individual pipe assets (several site locations have multiple pipe assets), including: o Replacement of approximately 3,400 LF of sewer and storm pipe, ranging in diameter from 6" up to 24", with ductile iron or PVC, depending on depth, with couplings for instances of partial pipe replacement o Replacement or installation of new structures, all City of Bellevue Standard Types o CIPP Liner installation in gravity storm and sewer pipe, approximately 3,600 LF oCCTV inspection of Sewer and Storm pipe assets, both before and after construction "Cleaning of sewer and storm pipe before pre-construction CCTV inspection o Traffic control, TESC, pavement, curb and gutter, driveway apron, and landscaping restoration as needed to support the above activities ' Survey of storm and sewer manholes, catch basins, and cleanouts as well as pipe inverts postconstruction o Record Documents (markedup set of plans to show actual installation and removals that vary from that shown on design plans) Each bid shall be accompanied by a bid bond in the form of cash, certified check, cashier's check or surety bond in an amount equal to at least 5% of the amount of such bid. Checks shall be made payable to the City Treasurer. Should the successful bidder fail to enter into a contract and furnish a satisfactory performance and payment bond within the time stated in the specifications, the bid bond shall be forfeited to the City of Bellevue Supplemental Responsible Bidder Criteria will apply to this project, and are included in the Special Contract Information section, Item 17. In a timely manner, Bidders may request, in writing, that the City modify the supplemental criteria. The City reserves the right to modify or not modify the supplemental criteria in its discretion. The City shall issue an Addendum, prior to the Bid Due Date, should it modify the supplemental criteria. The City shall not modify the supplemental criteria after the Bid Due Date, therefore, Bidders are encouraged to discuss with the project manager immediately and before the Bid Due Date the relevancy ofthe supplemental bidder criteria. Any bidder determined to be not responsible based on these supplemental criteria may appeal the City's determination by providing additional information to the City within two 2 business days following receipt ofthe City's written determination that the bidder was determined not responsible. The successful bidder must comply with the City of Bellevue equal opportunity requirements.
