Site work and paving for a road / highway in Bethlehem, Connecticut. Working plans call for site work for a road / highway; and for paving for a road / highway.
Provide services to repair and pave a section of Auncient Oak Road in Bethlehem. The contractor is responsible to provide supervision and coordination services and supply all materials, labor, tools, equipment, and incidentals to perform repair and paving in accordance with the following Scope of Work and Bid Specifications. This work shall be performed on various roads in the Town of Bethlehem, in accordance with State of Connecticut DOT specifications for Roads, Bridges and Incidental Construction. The successful bidder must furnish a current Certificate of Insurance. No work shall commence until a meeting for scheduling has been agreed upon between the contractor and Town of Bethlehem. 1. Removal of approximately 2000 square feet of roadway and repair of subsurface material. (Contractor shall field determine this amount based on their own measurements.) Existing curbing shall remain intact. Contractor will perform fine grading and site preparation for vibratory rolling and paving. Contractor shall compact the surface using a vibratory roller. 2. Road Surface will consist of 2 courses: o 1 1/2 " Binder course of HMA .5 Superpave Asphalt o 1 1/2 " Wearing course of HMA .375 Superpave Asphalt o All thickness of paving are for post compaction and shall be field verified. o All equipment and labor necessary for key cuts to the starting and ending points and apply various thickness of asphalt. o All signage and traffic control devices necessary to ensure maintenance roadway safety unless otherwise specified by the Public Works Director or his/her designee. o A site visit with the Public Works Director or his/her designee to the project location, a minimum of two (2) days before the scheduled start dates. This site visit will be made so that all parties agree to the type of work to be performed at the job site location. o The contractor shall maintain local access through the project area at all times. o The contractor shall at the end of each workday ensure that the project area is left in a safe and passable condition. o The contractor will remove residual spoils related to the project. Any questions regarding this quotation shall be directed to Stephen Mantz, Director of Public Works, or his designee at (203) 266-7510 ext. 302. The Respondent shall, at its own expense and cost, obtain and keep in force during the entire duration of the Contract or Work the following insurance coverages covering the Respondent and all of its agents, employees and sub-contractors and other providers of services and shall name the Town of Bethlehem and their employees and agents as an Additional Insured on a primary and non-contributory basis to all policies except Workers Compensation and Professional Liability. These requirements shall be clearly stated in the remarks section on the Respondent's Certificate of Insurance. All policies should also include a Waiver of Subrogation. Insurance shall be written with Carriers approved in the State of Connecticut and with a minimum AM Best's Rating of "A-" VIII. In addition, all Carriers are subject to approval by the Town of Bethlehem. *The list of planholders and/or interested bidders is not being tracked by the stakeholders involved with this project.
